皮层抑制功能与腰椎间盘突出症慢性疼痛的相关性研究
Correlation Analysis between Cortical Inhibition Function and Chronic Pain Associated with Lumbar Disc Herniation

Abstract
Objective To explore the correlation between cortical silent period(CSP)and chronic pain in patients with lumbar disc herniation(LDH),and to provide an objective,sensitive and accurate indicator for chronic pain assessment in clinical practice.Methods A total of 61 patients with LDH treated in the Shanghai Tongji Hospital from March to December 2023 were recruited and divided into mild pain group and moderate pain group according to the Numerical Rating Scale(NRS)scores,with 22 and 39 cases in each group,respectively.Cortical excitability parameters[resting motor threshold(rMT)and CSP]and surface electromyog-raphy(sEMG)signals,including time-domain indices[average EMG(AEMG),root mean square(RMS)and integrated EMG(iEMG)]and frequency-domain indices[mean power frequency(MPF)and median frequency(MF)]between the two groups were compared.Additionally,the correlations between CSP and sEMG parameters were analyzed.Results(1)rMT and CSP:compared with the mild pain group,the CSP on the herniated side in the moderate pain group was significantly lower,with a statistically signifi-cant difference(P<0.05),while there were no statistically significant differences in CSP on the non-herniated side and rMT on both sides between the two groups(P>0.05).(2)sEMG:compared with the same side in the mild pain group,the iEMG,AEMG and RMS values in the moderate pain group were significantly lower(P<0.05),while MPF and MF were significantly higher(P<0.05).(3)Correlation between CSP and sEMG:bilateral CSP was positively correlated with time-domain sEMG parameters(iEMG,AE-MG,RMS)(P<0.05).Conclusion Chronic pain of LDH patients is associated with altered cortical inhibition,and CSP is closely related to both the severity of chronic pain and muscle function,suggesting its potential as an objective electrophysiological marker for pain assessment in LDH patients.关键词
