目的:通过评估几何外形不同,合金相和运动方式均相同的镍钛锉系统对3D打印严重弯曲根管模块的预备效果,研究根管镍钛锉几何外形对弯曲根管预备的影响.方法:将弯曲度为30°、50°的3D打印严重弯曲根管模块各20个平均分为4组(30° U-Gold组、30° X-Gold组、50° U-Gold组、50° X-Gold组),进行根管预备,CBCT扫描,三维重建.将分离出的根管与对应弯曲度的标准根管模型进行数字化比对,计算清洁度与偏移量,进行统计分析.结果:X-Gold镍钛锉对30°、50°弯曲根管的清洁度均高于U-Gold镍钛锉(P<0.05),在根管弯曲内侧的过度预备量少于U-Gold镍钛锉,在根管弯曲外侧的欠预备量少于U-Gold镍钛锉;在30°根管中1/3段和下1/3段的偏移量均小于U-Gold镍钛锉(P<0.05),在50°根管中1/3段和下1/3段的偏移量均小于U-Gold镍钛锉(P>0.05),但无统计学差异.结论:根管镍钛锉的几何外形对严重弯曲根管预备的清洁度和偏移量均有影响.偏轴心、矩形横截面的镍钛器械比同轴心、改良三角形横截面的镍钛器械对弯曲根管的清洁度高,且在根管中下1/3段产生的偏移量小,更适合严重弯曲根管的预备.

Objective:To investigate the effect of root canal NiTi instruments geometry on the preparation of curved root canals by evaluating the effect of NiTi instrumentation systems with different geometrical and identical alloy phases and kinematics on the preparation of 3D printed significantly curved root canals.Methods:Twenty 3D printed root canal prepara-tion modules with 30° curvature and 20 with 50° curvature were divided equally into four groups(30° U-Gold group,30° X-Gold group,50° U-Gold group,50° X-Gold group).These groups were subjected to root canal preparation,CBCT scanning,3D reconstruction.The separated root canals were digitally compared with a standard root canal model of the corresponding curvature,cleanliness and transportation were calculated,and statistical analysis was performed.Results:X-Gold cleaned the 30° and 50° curved root canals better than the U-Gold(P<0.05),with less over-preparation on the inside of the curved root canal than the U-Gold and less under-preparation on the outside of the curved root canal than the U-Gold.The transportation in the middle 1/3 and apical 1/3 of the 30° root canal were less than the U-Gold(P<0.05),and in the 50° root canal the transportation in the middle 1/3 and apical 1/3 segments were smaller than in the U-Gold(P>0.05),but not statistically dif-ferent.Conclusion:The geometry of the root canal NiTi instruments has an effect on both cleanliness and transportation of se-verely curved root canal preparations.NiTi instruments with an off-axis,rectangular cross-sectional geometry are more suitable for the preparation of severely curved root canals because they clean the curved canals better and produce less deflection in the middle and apical third of the canal than NiTi instruments with a coaxial,modified triangular cross-section.
