经胸超声心动图与经食管超声心动图对卵圆孔未闭的诊断效能对比
Comparison on Diagnostic Efficacy of Transthoracic Echocardiography and Transesophageal Echocardiography for Patent Foramen Ovale

Abstract
Objective To compare and analyze the diagnostic efficacy of transthoracic echocardiography(TTE)and transesophageal echocardiography(TEE)for patent foramen ovale(PFO).Methods A total of 80 patients with suspected PFO admitted to our hospital from June 2023 to June 2024 were selected as the study subjects.All patients underwent TTE and TEE examinations.The results of agitated saline contrast echocardiography(ASCE)were used as the"gold standard",to compare the efficacy of TTE and TEE examination in the diagnosis of PFO(sensitivity,accuracy,specificity,etc.),consistency with the gold standard(Kappa test)and the incidence of complications.Results The result of ASCE examination showed that among the 80 patients,56 cases were diagnosed with PFO.The sensitivity,accuracy,specificity,positive predictive value and negative predictive value of TEE examination for the diagnosis of PFO were 96.43%(54/56),96.25%(77/80),95.83%(23/24),98.18%(54/55)and 92.00%(23/25),respectively,higher than those of TTE examination[82.14%(46/56),77.50%(62/80),66.67%(16/24),85.19%(46/54)and 61.54%(16/26)],with statistically significant difference(all P<0.05).The results of the consistency analysis showed that the consistency between TTE examination result and gold standard was moderate(Kappa=0.477,P<0.05),and the consistency between TEE examination result and gold standard was good(Kappa=0.912,P<0.05).No significant difference was found in the total complication rate(1.25%vs.6.25%)between TTE and TEE examination(x2=1.558,P=0.212).Conclusions Compared with TTE examination,TEE examination has higher efficacy in diagnosing PFO.In clinical practice,it is recommended to give priority to TTE examination for patients with suspected PFO,and further TEE examination for positive or suspicious PFO patients.关键词
