颈部超声联合MRI血管成像在脑梗死颈部血管狭窄性病变诊断中的应用价值
The Application Value of Neck Ultrasound Combined with MRI Angiography in the Diagnosis of Cervical Vascular Stenotic Lesions in Cerebral Infarction

Abstract
Objective To explore the application value of neck ultrasound combined with magnetic resonance imaging(MRI)angiography in the diagnosis of cervical vascular stenotic lesions in cerebral infarction.Methods A total of 103 patients with suspected cervical vascular stenotic lesions in cerebral infarction admitted to the Fifth Affiliated Hospital of Zhengzhou University from January 2022 to December 2024 were selected as the research objects,and all of them underwent cervical ultrasound and MRI angiography after admission.The results of digital subtraction angiography(DSA)were used as the gold standard,to analyze the detection results and diagnostic efficacy of cervical ultrasound and MRI angiography alone and in combination on cervical vascular stenotic lesions in cerebral infarction.Results DSA examination results showed that among the 103 patients,13 cases(12.62%)had normal neck blood vessels and 90 cases(87.38%)had cervical vessel stenosis.With the results of DSA as the gold standard,the Kappa index of cervical ultrasound in the diagnosis of cervical vascular stenotic lesions in cerebral infarction was 0.756,with good consistency.The Kappa index of MRI angiography diagnosis was 0.761,with good consistency.The Kappa index of cervical ultrasound combined with MRI angiography diagnosis was 0.801,with good consistency.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of cervical ultrasound in the diagnosis of cervical vascular stenotic lesions in cerebral infarction were 79.61%,78.89%,84.62%,97.26%and 36.67%,respectively.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of MRI angiography were 87.38%,86.67%,92.31%,98.73%and 50.00%,respectively.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of cervical ultrasound combined with MRI angiography were 96.12%,97.78%,84.62%,97.78%and 84.62%,respectively.The accuracy,sensitivity and negative predictive value of cervical ultrasound combined with MRI angiography in the diagnosis of cervical vascular stenotic lesions in cerebral infarction were higher than those of cervical ultrasound and MRI angiography alone(P<0.05).Conclusions Cervical ultrasound and MRI angiography have certain detection effect on cervical vascular stenotic lesions in cerebral infarction,but there are certain limitations.The combination of the two examination methods can improve the accuracy,sensitivity and negative predictive value in the diagnosis of cervical vascular stenotic lesions in cerebral infarction.关键词
