糖尿病"三师共管"门诊诊疗2型糖尿病多中心临床研究
Multicenter Clinical Study on Co-Management under Three Disciplines Outpatient Service For Metabolic Indicators of Type 2 Diabetes Patients

Abstract
Objective To analyze the impact of the diabetes"co-management under three disciplines"outpatient service involving Chinese medicine(CM)practitioners on metabolic indicators in patients with type 2 diabetes mellitus(T2DM),and to evaluate the efficacy and advantages of this model in diabetes management.Methods From October 2021 to December 2022,T2DM patients attending the co-management outpatient service at the First Affiliated Hospital of Xiamen University and its affiliated centers were enrolled.A total of 100 patients were screened and randomly assigned to the co-management group(50 cases)and the routine care group(50 cases).The co-management group received a three-month multidisciplinary team intervention.Endocrinologists developed individualized medication regimens.Chinese medicine(CM)practitioners administered syndrome differentiation-based treatments,including herbal adjustments,symptom management and CM-specific dietary/exercise guidance.Health managers provided full-term lifestyle supervision.The routine care group received only standard medication management and health education from endocrinologists.Metabolic indicators[glycated hemoglobin(HbA1c),low-density lipoprotein cholesterol(LDL-C),triglycerides(TG),total cholesterol(TC),fasting blood glucose(FBG)],systolic blood pressure and quality of life[European Quality of Life-5 Dimensions-5 Level(EQ-5D-5L)Chinese version score]were compared between the two groups before and after the intervention.Results Compared with before intervention,the level of HbA1c,FBG and systolic blood pressure in the co-management group and the routine care group decreased(P<0.05),and the score of the EQ-5D visual analogue scale increased(P<0.05).The LDL-C level in the co-management group decreased(P<0.05).Compared with the routine care group,the differences in HbA1c and LDL-C before and after intervention in the co-management group were greater(P<0.05),and the level of FBG decreased(P<0.05).Conclusions The"co-management under three disciplines"outpatient service significantly optimized HbA1c and LDL-C levels,demonstrating superior metabolic control compared to routine care.However,no short-term improvement in quality of life was observed.(Chinese Clinical Trial Registry,No.ChiCTR2300075465).关键词
