Multicenter Cross-Sectional Study on Coronary Heart Disease Combined with Cognitive Impairment

Abstract
Objective To explore the characteristics of diseases and Chinese medicine(CM)syndromes,and summarize core medications and prescriptions by a cross-sectional study of disease-syndromes-medicine on patients with coronary heart disease(CHD)and cognitive impairment on real-world data.Methods This study included 545 inpatients with CHD and cognitive impairment from real-world database of cardiovascular diseases treated with CM.General information,CM syndrome elements,and the use of medications were collected.Disease diagnosis and CM syndrome element information were analyzed,and cluster and correlation analysis were performed on the application of Chinese herbal medicine.Results The syndrome elements were mainly phlegm,blood stasis,and qi deficiency,and the symptom types mainly included wind-phlegm obstructing the collaterals syndrome,phlegm-stasis inter-conjugation syndrome,and qi deficiency and blood stasis syndrome in patients with CHD combined with cognitive impairment.Medication use found that Donepezil was the most frequently used western drug to improve cognitive function.Among the proprietary Chinese medicines for the treatment of CHD or cognitive function-related diseases,Compound Congrong Yizhi Capsule,Zhongfeng Huichun Tablets and Ginkgo Biloba Tablets were used more frequently.Totally 95 effective Chinese medicine prescriptions were extracted,including 17 high-frequency Chinese herbal medicines with a frequency of ≥20％,and 7 Chinese herbal medicines with a frequency of more than 30 times,from high to low,which were Poria,Ligusticum Chuanxiong,Gastrodia,Salvia Miltiorrhiza,Glycyrrhizae Radix Et Rhizoma,Angelica Sinensis,and Acorus Tatarinowii.Cluster analysis of high-frequency herbs resulted in 4 drug combinations,with"Poria,Codonopsis""Angelica Sinensis,Astragalus""Gastrodia,Uncaria""Ligusticum Chuanxiong,Paeonia Rubra"and"Acorus Tatarinowii,Salvia Miltiorrhiza,Curcuma"showing high correlation.Analysis of the combination of Chinese and Western medicines showed that Donepezil and Cytarabine Sodium Tablets were often used in combination with proprietary Chinese patent medicines such as Zhongfeng Huichun Tablets and Nao'an Granules,and that Chinese herbal prescriptions were often prescribed with Salvia Miltiorrhiza,Gastrodia,Ligusticum Chuanxiong,etc.Conclusions Wind-phlegm obstructing the collaterals,phlegm and blood stasis intermingling,and qi deficiency and blood stasis are the main syndromes in CHD combined with cognitive impairment patients.CM classic formulas combined with Chinese and Western proprietary medicines such as Donepezil and Zhongfeng Huichun Tablets could be used clinically.When combined Chinese and Western treatments are used,Donepezil and Zhongfeng Huichun Tablets are most commonly used,while Chinese herbal prescriptions are often accompanied by Salvia Miltiorrhiza,Gastrodia,Ligusticum Chuanxiong,etc.关键词
