扶肾颗粒干预Vsig4驻留巨噬细胞治疗腹膜透析相关性腹膜炎随机对照试验
Treatment of Peritoneal Dialysis-associated Peritonitis with Fushen Granule Intervention of Vsig4 Resident Macrophages:A Randomized Controlled Trial

Abstract
Objective To observe the clinical efficacy and mechanisms of action of Fushen Granule in treating peritoneal dialysis-associated peritonitis(PDAP)through its intervention on Vsig4 resident macrophages.Methods This study evaluated the efficacy of Fushen Granule combined with antibiotics in the treatment of PDAP through a randomized controlled trial.Forty PDAP patients who visited the First Teaching Hospital of Tianjin University of Traditional Chinese Medicine from July 2023 to February 2024 were included and randomly divided into two groups,the Fushen Granule group and the control group with 20 patients in each group.The control group managed the patients' blood pressure and blood glucose level,and administered sensitive antibiotics intraperitoneally for 14 days.The Fushen Granule group,in addition to the treatment of the control group,received combined treatment with Fushen Granule,with each dose being 2 bags,twice a day,for a total of 14 days.The recovery time and ultrafiltration volume of patients were recorded.Peritoneal dialysis fluid samples were collected before and after PDAP treatment to detect changes in peritoneal Vsig4 macrophages,T cells,and cytokines interleukin(IL)-6,IL-10,and tumor necrosis factor-α(TNF-α).Results The average recovery time of patients in the Fushen Granule group was(2.50±0.26)days,which was significantly shorter than that of the control group[(3.30±0.25)days,t=-2.223,P=0.032)].On the third day of treatment,the ultrafiltration volume in the Fushen Granule group was significantly higher than the control group(P＜0.05).Compared with the baseline levels before treatment in their respective groups,the concentrations of IL-6,IL-10,and TNF-αsignificantly reduced in both groups(P＜0.01).The levels of CD3+T cells,CD14+macrophages,Vsig4+macrophages,and the ratio of Vsig4+macrophages to CD14+macrophages significantly increased in the Fushen Granule group(P＜0.05,P＜0.01),while the levels of CD3+T cells,Vsig4+macrophages,and the ratio of Vsig4+macrophages to CD14+macrophages also significantly increased in the control group(P＜0.05,P＜0.01).Compared with the control group,the proportion of Vsig4+macrophages and the concentration of IL-10 were significantly higher in the Fushen Granule group(P＜0.05),while the concentration of IL-6 was significantly lower(P＜0.01).Conclusions Early application of Fushen Granule in patients with PDAP can shorten the recovery time.The mechanism may be related to its ability to regulate the proportion of Vsig4 resident macrophages and the concentration of cytokines,thereby alleviating the peritoneal inflammatory response.关键词
