Abstract
Infiltrating epitheliosis(IE)of the breast is a rare complex sclerosing lesion characterized by the infiltration of ductal epithelial cells into a sclerotic elastic stroma,accompanied by usual-type ductal hyperplasia and loss of myoepithelial cells around the proliferative epithelial nests.From July 2021 to December 2023,two cases of IE were diagnosed at Suzhou Hospital Affiliated to Nanjing Medical University.Both patients were female,aged 53 and 47 years,respectively.Case 1 presented with a left breast mass detected during a physical examination,occasionally accompanied by nipple discharge.Case 2 had a right breast mass identified during a physical exam eight years earlier,with no significant change in size since.Histologically,both lesions showed florid ductal epithelial hyperplasia.In some areas,epithelial cells extended into the sclerotic stroma in irregular,cord-like patterns,forming a pseudo-infiltrative appearance.The nuclei displayed fine chromatin with variable size and shape but lacked significant atypia.Immunochemistry revealed that the proliferating ductal epithelium was positive for estrogen receptor(ER)and progesterone receptor(PR)with variable intensity,and diffusely positive for cytokeratin(CK)5/6 and CK14.Myoepithelial markers including calponin,p63,smooth muscle actin(SMA),and myosin showed loss of expression around the proliferating epithelial nests.Genetic testing demonstrated PIK3CA mutations in both cases.This report underscores the need for pathologists to recognize IE in order to avoid misdiagnosis as invasive carcinoma or ductal carcinoma in situ.关键词
