Abstract
Objective To observe the clinical efficacy and safety of budesonide suspension for inhalation combined with cefoperazone sulbactam for injection in the treatment of neonatal pneumonia.Methods The children with neonatal pneumonia were divided into treatment group and control group according to the cohort method.Treatment group was treated with cefoperazone sulbactam for injection 30 mg·kg-1,twice a day,combined with budesonide suspension 1 mg,twice a day,while control group was treated with cefoperazone sulbactam for injection 30 mg·kg-1,twice a day.The clinical efficacy,improvement time of clinical symptoms and signs,blood routine examination,inflammation level and blood gas analysis indexes were compared between the two groups,and the safety was evaluated.Results A total of 102 children were included in this study,including 51 cases in treatment group and 51 cases in control group.After 7 days of treatment,the total clinical effective rate of the treatment group was 98.04%(50 cases/51 cases),which was higher than 84.31%(43 cases/51 cases)of control group(P<0.05).After treatment,the disappearance time of pulmonary moist rales in treatment group and control group were(4.31±0.65)and(6.02±0.81)days,respectively;the disappearance time of shortness of breath were(3.29±0.54)and(4.65±0.77)days;the disappearance time of cough were(3.86±0.57)and(5.25±0.77)days;the recovery time of body temperature were(2.43±0.54)and(3.63±0.63)days,respectively.Compared with control group,the above indexes in treatment group were significantly shortened(all P<0.05).The white blood cell count of treatment group and control group were(8.89±2.14)and(12.06±3.97)× 109·L-1,respectively;the ratio of neutrophils to lymphocytes were 1.11±0.19 and 1.35±0.25,respectively;the serum C-reactive protein were(7.44±1.40)and(11.24±2.62)mg L-1,respectively;procalcitonin were(0.76±0.14)and(1.14±0.22)μg L-1,respectively;the partial pressure of carbon dioxide(PaCO2)were(41.29±4.94)and(45.50±5.55)mmHg,respectively.Compared with control group,the levels of the above indicators in treatment group were significantly lower(all P<0.05).The partial pressure of oxygen(PaO2)in treatment group and control group were(89.25±9.28)and(83.72±7.89)mmHg,respectively;the blood oxygen saturation(SpO2)levels were(95.47±4.75)%and(87.82±4.02)%respectively,which were significantly higher in treatment group than those in control group(all P<0.05).The main adverse drug reactions in treatment group were nausea and vomiting and rash,while those in control group were nausea and vomiting.The total incidence of adverse drug reactions in treatment group was 5.88%(3 cases/51 cases),and that in the control group was 3.92%(2 cases/51 cases).There was no significant difference between the two groups(P>0.05).Conclusion Compared with the single use of cefoperazone sulbactam for injection,combination of budesonide suspension for inhalation in the treatment of neonatal pneumonia can improve the curative effect,shorten the time of symptom improvement,improve blood routine,inflammation level and blood gas analysis index and has good safety.关键词
