蝉花散联合梅花针治疗肝经风热型青光眼的临床研究
Clinical Study on Chanhua Powder Combined with Plum-blossom Needling for Pro-tecting Optic Nerve and Visual Function in Glaucoma Patients with Liver Meridian Wind-heat Pattern

Abstract
OBJECTIVE To observe the therapeutic effect of Chanhua Powder combined with Plum-blossom Needling on glaucoma patients with Liver Meridian Wind-heat Pattern.METHODS A total of 80 patients(132 eyes)diagnosed with glaucoma and Liver Meridian Wind-heat Pattern at the Ophthalmology Clinic of Urumqi Traditional Chinese Medicine Hospital from January 2022 to December 2023 were enrolled.They were randomly divided into a control group(CG,40 cases,67 eyes)and an observation group(OG,40 cases,65 eyes).The control group received conventional intraocular pressure-lowering eye drops,while the observation group received additional Chanhua Powder combined with Plum-blossom Needling,both treated 60 days.TCM syndrome scores,intraocular pressure(IOP),retinal nerve fiber layer(RNFL)thickness,visual field indices[mean defect(MD)],mean sensitivity(MS)],pattern visual evoked potential(P-VEP),and vision-related quality of life were observed before and after treatment.RESULTS(1)TCM Symptom Scores:Scores significantly decreased in both groups post-treatment compared to pre-treatment(tOG=17.054,tCG=10.714,both P=0.000).Post-treatment scores in the observation group were significantly lower than those in the control group(t=10.626,P=0.000).(2)IOP:IOP significantly decreased in both groups post-treatment compared to pre-treatment(tOG=11.396,tCG=6.277,both P=0.000).Post-treatment IOP in the observation group was significantly lower than that in the control group(t=5.520,P=0.000).(3)RNFL Thickness:RNFL thickness significantly decreased in both groups post-treatment compared to pre-treatment(tOG=9.689,tCG=5.535,both P=0.000).Post-treatment RNFL thickness in the observation group was significantly lower than that in the control group(t=4.898,P=0.000).(4)Visual Field Indices:MD significantly decreased in both groups post-treatment compared to pre-treatment(tOG=13.038,tCG=7.075,both P=0.000).MS significantly increased in both groups post-treatment compared to pre-treatment(tOG=6.279,tCG=3.633,both P=0.000).Post-treatment,the observation group had significantly lower MD(t=2.874,P=0.005)and significantly higher MS(t=3.735,P=0.000)than the control group.(5)P-VEP:Amplitude significantly increased in both groups post-treatment compared to pre-treatment(tOG=24.541,tCG=12.589,both P=0.000).Latency significantly shortened in both groups post-treatment compared to pre-treatment(tOG=10.134,tCG=4.199,both P=0.000).Post-treatment,the observation group had significantly higher P-VEP amplitude(t=12.374,P=0.000)and significantly shorter latency(t=6.750,P=0.000)than the control group.(6)Vision-Related Quality of Life Scores:Scores for General Health&Vision(tOG=18.635,tCG=8.465,both P=0.000),Activity Limitations(tOG=14.538,tCG=7.752,P=0.000),and Vision-Specific Difficulties(tOG=19.355,tCG=10.214,both P=0.000)significantly increased in both groups post-treatment compared to pre-treatment.Post-treatment,the observation group had significantly higher scores than the control group for General Health&Vision(t=6.991,P=0.000),Activity Limitations(t=7.752,P=0.000),and Vision-Specific Difficulties(t=8.954,P=0.000).(7)Clinical efficacy:The total clinical effective rate was 93.84%in the observation group versus 77.61%in the control group,with a statistically significant difference(χ2=7.057,P=0.008).CONCLUSIONS Chanhua Powder combined with Plum-blossom Needling effectively alleviates clinical symptoms,reduces IOP,improves visual fields,and mitigates optic nerve damage in glaucoma patients with Liver Channel Wind-heat Pattern.The mechanism may relate to modulating visual field photosensitivity and P-VEP latency/amplitude.关键词
