肝细胞癌合并微血管侵犯的增强MRI影像学特征及其影响因素分析
Enhanced MRI imaging features of hepatocellular carcinoma combined with microvascular invasion and analysis of its influencing factors

Abstract
Objective To analyse the enhanced MRI imaging features of hepatocellular carcinoma(HCC)combined with microvascular invasion(MVI)and its influencing factors.Methods A total of 60 HCC patients admitted to Ganzhou Cancer Hospital from October 2022 to October 2023 were selected as the study subjects,and they were divided into the control group(32 cases)and MVI group(28 cases)according to whether they were combined with MVI or not.Enhanced MRI was performed in both groups,and the gold standard,that is,the pathological results,was analysed for imaging features.The independent influencing factors for the occurrence of MVI in HCC patients were analysed using univariate and multivariate logistic regression.Results The results of univariate analysis showed that the pathological grading,whether the tumour mar-gins were smooth,whether the peritumoural arteries were enhanced,whether there was fat in the tumour,and whether the peritumoural hyposignal appeared in the hepatobiliary stage were compared between the two groups,and the differences were statistically significant(P<0.05).The results of multifactorial logistic regression analysis showed that pathological grade,non-smooth tumour margins,peritumoural arterial enhancement,absence of intratumoural fat,and peritumoural low signal in hepatobiliary stage were the independent influencing factors for the development of MVI in patients with HCC(P<0.05).Conventional MRI signs:the intratumoral signals can be classified as homogeneous or heterogeneous.Heterogeneous signals include fatty degeneration,haemorrhage and necrosis.The envelope includes 3 categories:intact,incomplete,and not found.Significantly enhancing wedge-shaped and irregular shaped shadow around the tumour in the arterial phase.Hepato-biliary phase signs:the margins of the tumour in the hepatobiliary phase appear wedge-shaped,irregular,which is slightly lower than the signal shadow of normal liver parenchyma.Conclusion HCC combined with MVI has a significant effect on enhanced MRI diagnosis,which can provide effective data reference for clinical differential diagnosis and subsequent treatment plan,and has popularization value.关键词
