Social isolation trajectory classification and its impact on loneliness in el-derly patients with type 2 diabetes mellitus

Abstract
Objective Based on the latent variable growth mixed model,the trajectory classification of social isolation in elderly patients with type 2 diabetes mellitus was analyzed,and its impact on the patients' sense of loneliness was explored.Methods A retrospective analysis was conducted on the clinical data of 280 elderly patients with type 2 diabetes mellitus admitted to the First People's Hospital of Fuzhou City from June 2021 to December 2023.The Lubben social network scale-6 was used to evaluate the degree of social isolation of the patients during the first treatment,at 1,3,and 6 months of treatment.The optimal number of categories was analyzed and named through the growth mixed model.The University of California Los Angeles loneliness scale-8(ULS-8)was used to assess the loneliness of patients,and multiple linear regression was used to analyze the influencing factors of loneliness in elderly patients with type 2 diabetes mellitus.Results The social isolation trajectories of 280 elderly patients with type 2 diabetes mellitus could be classified into 50 cases(17.86％)with high social isolation,162 cases(57.86％)with reduced social isolation,and 68 cases(24.28％)with low social isolation.The average score of ULS-8 of the patients was(22.34±3.52)points.The results of univariate analysis showed that elderly diabetic mellitus patients who were unmarried/divorced/widowed,lived alone,had high school education or lower,never exercised,had fasting blood glucose of≤7.0 mmol/L,and had a moderate or higher degree of depression had a higher sense of loneliness,the sense of loneliness of high-type patients was higher than that of reduced-type and low-type patients,and that of reduced-type patients was higher in the social isolation trajectory,the differences were statistically significant(P＜0.05).The results of multivariate analysis conducted by the multiple linear regression model showed that being unmarried/divorced/widowed(B=1.256,t=3.773,95％CI:0.601-1.912)was an independent risk factor for loneliness in elderly patients with type 2 diabetes mellitus,and had college degree or above(B=-1.663,t=-5.220,95％CI:-2.290 to-1.036),regular exercise(B=-0.971,t=-3.126,95％CI:-1.582 to-0.359),fasting blood glucose level＞7.0 mmol/L(B=-1.267,t=-3.961,95％CI:-1.897 to-0.637),mild depression or less(B=-0.658,t=-2.346,95％CI:-1.217 to-0.099),low social isolation type(B=-2.118,t=-8.978,95％CI:-2.583 to-1.654)were independent protective factors for loneliness in elderly patients with type 2 diabetes mellitus(P＜0.05).Conclusion There is a certain heterogeneity in the types of social isolation trajectories of elderly patients with type 2 diabetes mellitus,and the low type of social isolation is a protective factor for the patients' sense of loneliness.关键词
