超声引导下囊周神经丛阻滞联合坐骨神经阻滞对老年患者全髋关节置换术后疼痛程度及疼痛介质释放的影响
Effect of ultrasound-guided pericapsular nerve group block combined with sciatic nerve block on the degree of pain and the release of pain me-diators in elderly patients after total hip replacement

Abstract
Objective To investigate the effect of ultrasound-guided pericapsular nerve group(PENG)block combined with sciatic nerve block on the degree of pain and the release of pain mediators in elderly patients after total hip replacement.Methods A total of 110 elderly patients with hip injury who underwent total hip replacement in the First Hospital of Nan-chang from April 2022 to April 2024 were selected as the research objects.They were divided into control group(55 cases)and observation group(55 cases)according to the random number table method.The control group was treated with ultrasound-guided PENG block,and the observation group was treated with ultrasound-guided PENG block combined with sciatic nerve block.The anesthetic effect,postoperative visual analogue scale(VAS)score,inflammatory pain mediators(prostaglandin E2[PGE2],substance P[SP],β-endorphin[β-EP])levels and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,and the differ-ence was statistically significant(P＜0.05).The VAS scores of the two groups at each time point after operation were lower than those before operation,and the VAS scores of the observation group at each time point after operation were lower than those of the control group,and the differences were statistically significant(P＜0.05).The PGE2 and SP levels of the two groups on the 1st,3rd and 7th day after operation were lower than those before operation,and the β-EP levels were higher than those before operation,and the PGE2 and SP levels of the observation group on the 1st,3rd and 7th day after operation were lower than those of the control group,and the β-EP levels were higher than those of the control group,the differences were statistically significant(P＜0.05).There was no statistically significant difference in the total incidence of adverse re-actions between the two groups(P＞0.05).Conclusion For elderly patients undergoing total hip replacement,the application of ultrasound-guided PENG block combined with sciatic nerve block can regulate the levels of PGE2 and SP,which is bene-ficial to relieve the pain and discomfort of patients.关键词
