基于Kano模型的泌乳启动延迟产妇维持母乳喂养的支持性照护需求分析
Analysis of supportive care needs for maintaining breastfeeding in par-turient with delayed lactation initiation based on the Kano model

Abstract
Objective Based on the Kano model,to analyze the supportive care needs of parturient with delayed lactation initiation for maintaining breastfeeding,further screen the needs and optimize the nursing strategies.Methods A total of 200 parturient with delayed lactation initiation admitted to Ganzhou Women and Children's Health Care Hospital from January 2022 to June 2024 were selected as the research subjects.A questionnaire on the supportive care needs for maintaining breastfeeding in parturient with delayed lactation initiation was designed using the Kano model combined with the Delphi expert consultation method,and two rounds of inquiries were conducted.A total of six obstetrics and gynecology experts evaluated the rationality,comprehensiveness and applicability of each indicator in the questionnaire.They modified the questionnaire indicators based on the experts' opinions and made further adjustments after conducting a pre-survey.Eventually,a questionnaire consisting of 16 items across 5 dimensions was formed.The validity of the questionnaire was tested using the KMO test method.Through the classification of demand attributes in the Kano model,Better-Worse coefficient analysis and two-dimensional matrix analysis,the information demand attributes were determined,and the most sensitive and most in need of improvement elements were screened out through the demands for strategy optimization.Re-sults The positive coefficient of all experts in the two rounds of inquiries was 100.00％,and the authority coefficient was 0.845.The coordination coefficients of the two rounds of expert opinions were 0.426 and 0.457 respectively(P＜0.05).In this survey,the reliability and validity of the questionnaire were good,with Cronbach's＞0.800.The overall KMO test coeffi-cients were all＞0.5,and the validity of the questionnaire was good.Among the 16 items in the questionnaire survey,14(87.50％)need to be prioritized for improvement,including 2 items of charm needs,7 items of essential needs,and 5 items of expected needs."Early feeding plan formulation","increasing opportunities for mother-infant interaction",and"diet control"were the three most sensitive needs.Conclusion Based on the Kano model,the supportive care needs for maintain-ing breastfeeding in parturient with delayed lactation initiation that urgently need improvement were screened out,and the optimization strategies for the supportive care needs of such patients were proposed according to the sensitivity values.Providing relevant support for maintaining breastfeeding,strengthening the intervention of supportive care,helping patients build reasonable postoperative rehabilitation habits and good mother-infant relationships,and preventing adverse maternal and infant outcomes during this process,thereby meeting the supportive care needs of such patients.关键词
