内窥镜逆行胰胆管造影联合腹腔镜胆囊切除术在胆囊结石合并肝外胆管结石治疗中的应用效果
Application effect of endoscopic retrograde cholangiopancreatography combined with laparoscopic cholecystectomy in the treatment of gallstones with extrahepatic bile duct stones

Abstract
Objective To explore the effect of endoscopic retrograde cholangiopancreatography(ERCP)combined with la-paroscopic cholecystectomy(LC)in the treatment of gallstones(GS)combined with extrahepatic bile duct stones.Methods A total of 82 patients with GS combined with extrahepatic bile duct stones admitted to the Second Affiliated Hospital of Mu-danjiang Medical University from November 2020 to November 2023 were retrospectively selected as the study subjects,and they were divided into the hybrid surgery group(n=44)and the control group(n=38)according to the difference in surgical methods.The hybrid surgery group was treated with ERCP combined with LC hybrid surgery,and the control group was treated with conventional open surgery.Perioperative indices,serum biochemical indices,surgical success rate,recurrence rate and complication occurrence were compared between the two groups.Results The intraoperative bleeding in the hybrid surgery group was less than that in the control group,and the operation time and hospital stay were shorter than those in the control group,with statistically significant differ-ences(P＜0.05).At 3 d postoperatively,the levels of alanine transaminase(ALT),aspartate transaminase(AST),and serum amylase(AMY)in the hybrid surgery group were lower than those in the control group,and the differences were statistically significant(P＜0.05).There was no statistically significant difference in the success rate of surgery between the two groups(P＞0.05).The recurrence rate within 6 months in the hybrid surgery group was lower than that in the control group,and the difference was statistically significant(P＜0.05).The total complication rate of the hybrid surgery group was lower than that of the control group,and the difference was statistically significant(P＜0.05).Conclusion The combination of ERCP and LC surgery is more effective in the treatment of patients with GS complicated with extrahepatic bile duct stones,which can shorten the operation time and reduce intraoperative bleeding,and reduce the damage to liver and pancreatic function,with a lower risk of complications.Besides,the recurrence rate within 6 months is lower.关键词
