血清HIF-1α、Apelin水平变化与糖尿病性黄斑水肿程度的关系及对其发病风险的预测价值
The relationship between serum HIF-1α and Apelin levels with the degree of diabetic macular edema and its predictive value for risk of onset

Abstract
Objective To analyze the relationship between the expression of serum hypoxia-inducible factor-1α(HIF-1α)and Apelin and the disease severity in patients with diabetic macular edema(DME),and to explore the predic-tive value of serum HIF-1α and Apelin for the risk of DME.Methods Prospective case-control study.A total of 126 pa-tients with type 2 diabetes mellitus(T2DM)and DME who were treated in our hospital from August 2022 to May 2024(DME group)were enrolled.According to the degree of DME,the patients were grouped into mild group(53 cases),mod-erate group(48 cases)and severe group(25 cases).In addition,50 patients with T2DM who came to our hospital at the same time(T2DM group)were selected.The general information of the included objects was recorded,the fasting blood glu-cose(FPG),glycosylated hemoglobin(HbA1c),high/low-density lipoprotein cholesterol(H/LDL-C),central macular thickness(CMT),serum HIF-1α,and Apelin were measured.Pearson was used to analyze the correlation between HIF-1α,Apelin and CMT.Logistic regression analysis was used to analyze the influencing factors of DME.ROC was used to analyze the predictive value of serum HIF-1α and Apelin for the risk of DME.Results The course of T2DM,FPG,HbA1c,HIF-1α and Apelin in the DME group were longer than or higher than those in the T2DM group(all P<0.05).The HIF-1α,Apelin and CMT in the moderate group and severe group were higher than those in the mild group(all P<0.05),and the HIF-1α,Apelin and CMT in the severe group were higher than those in the moderate group(all P<0.05).Serum HIF-1α and Apelin were positively correlated with CMT(rHIF-1α=0.426,rApelin=0.402,P<0.05).Long duration of T2DM and high levels of FPG,HbA1c,HIF-1α and Apelin were risk factors for the onset of DME in patients with T2DM(all P<0.05).The AUC of serum HIF-1α,Apelin,and combined prediction of the onset of DME in patients with T2DM were 0.787,0.796,and 0.909,respectively.The sensitivity was 71.43%,78.57%,and 96.83%,and the specificity was 74.00%,72.00%,and 68.00%,respectively.The value of combined prediction was relatively high(Zcombination-HIF-1α=3.114,Zcombination-Apelin=2.779,P<0.05).Conclusions The serum levels of HIF-1α and Apelin in DME patients are higher,and their levels increase with the severity of DME.Serum HIF-1α and Apelin were the influencing factors of the risk of DME.Combined testing can predict the risk of DME to some extent.关键词
