Changes and clinical significance of IL-6,MCP-1 and TIMP-1 levels in aqueous humor of patients with type 2 diabetic cataract

Abstract
Objective To investigate the changes and clinical significance of interleukin-6(IL-6),monocyte che-moattractant protein-1(MCP-1)and matrix metalloproteinase inhibitor-1(TIMP-1)in aqueous humor of patients with type 2 diabetic cataract.Methods Retrospective case-control study.total of 96 patients with type 2 diabetic cataract admitted to our hospital from August 2020 to August 2024 were selected as the observation group,and 80 patients with simple cataract admitted to our hospital during the same period were selected as the control group.The clinical data,levels of IL-6,MCP-1 and TIMP-1 in aqueous humor were compared between the occurring group and the non-occurring group.Compare the dif-ference in anterior chamber flare between the occurring group and the non-occurring group after surgery,and perform a Spearman correlation analysis between postoperative anterior chamber flare and the levels of IL-6,MCP-1,and TIMP1.Lo-gistic multivariate regression analysis was used to analyze the risk factors of ME.The curve of receiver operating character-istics(ROC)was drawn to evaluate the predictive efficacy of IL-6,MCP-1,TIMP-1 and their combination in postoperative ME in patients with type 2 diabetic cataract.Results The levels of IL-6,MCP-1,and TIMP-1 in the aqueous humor of the observation group were higher(P<0.05).The degree of anterior chamber flare after surgery is positively correlated with the levels of IL-6,MCP-1,and TIMP-1.The incidence of ME in type 2 diabetes cataract patients was 29.46%(33/112).The course of diabetes,the levels of HbA1C,IL-6,MCP-1 and TIMP-1 in the occurrence group were higher(P<0.05).Logistic multi factor regression analysis showed that the course of diabetes(OR=2.866,95%CI:1.208~6.798),IL-6(OR=3.442,95%CI:1.451~8.164),MCP-1(OR=3.350,95%CI:1.412~7.946)and TIMP-1(OR=3.261,95%CI:1.375~7.735)were all risk factors affecting the occurrence of ME in patients with diabetes induced cataract after surgery(P<0.05).The ROC curve analysis results showed that the AUC of IL-6,MCP-1,TIMP-1,and their combination in predicting ME occurrence in aqueous humor were 0.847(95%CI:0.759~0.912),0.744(95%CI:0.645~0.828),0.783(95%CI:0.687~0.861),and 0.899(95%CI:0.820~0.951),respectively.Conclusions The levels of IL-6,MCP-1 and TIMP-1 in aqueous humor of patients with type 2 diabetic cataract are abnormally high,and the combination of the above three factors has high predictive efficiency for postoperative ME.The degree of anterior chamber flare in patients with type 2 diabetic cataract after surgery is positively correlated with the levels of IL-6,MCP-1,and TIMP-1 in the aqueous humor.关键词
