探讨0.01%阿托品、角膜塑形镜和两者联合使用对儿童青少年近视防控的疗效及对视功能的影响
Investigation on the efficacy of 0.01%atropine,orthokeratology lenses and their combined use in the preven-tion and control of myopia in children and adolescents and their influence on visual function

Abstract
Objective To assess the efficacy and visual function alterations of 0.01%atropine eye drops,ortho-keratology lenses,and their combination in myopic children and adolescents.Methods Prospective case-control study.A total of 102 myopic children and adolescents aged 8~14 who visited the Affiliated Hospital of Nanjing University of TCM from June 2022 to May 2024 were selected as the subjects.They were categorized into the group with ordinary spectacles combined with 0.01%atropine eye drops(A group),the orthokeratology lens-wearing group,which was further randomly divided into the orthokeratology lens combined with artificial tear placebo group(OK group)and the orthokeratology lens combined with 0.01%atropine eye drop group(AOK group).A one-year follow-up was accomplished.Results Ninety cases completed the follow-up.There was no significant difference in baseline data among the three groups(P>0.05).Dur-ing the follow-up,the axial length growth in group AOK was the slowest(Ftime=1683.74,P=0.000;F time*group=53.097,P=0.000;Fgroup=101.727,P=0.000).There was no significant difference in the change of spherical equivalent among the three groups(Ftime=34.264,P=0.000;Ftime*group=0.965,P=0.428;Fgroup=1.727,P=0.184).There was no significant difference in negative relative accommodation(Ftime=1.284,P=0.278;Ftime*group=0.612,P=0.748;Fgroup=0.095,P=0.909),There were significant differences in positive relative accommodation(Ftime=22.852,P=0.000;Ftime*group=7.091,P=0.000;Fgroup=3.757,P=0.027),amplitude of accommodation(Ftime=107.824,P=0.000;Ftime*group=20.557,P=0.000;Fgroup=5.721,P=0.005),and accommodation flexibility(Ftime=94.479,P=0.000;Ftime*group=23.947,P=0.000;Fgroup=8.781,P=0.000)among the groups.Conclusions The combination of 0.01%atropine and OK lenses can achieve better myopia prevention and control,but may lead to a decrease in binocular accommodation.The study results provide a clinical basis for optimizing myopia preven-tion and control strategies in children and adolescents.关键词
