CRSwNP合并哮喘患者鼻内镜手术疗效的影响因素分析
Analysis of Influencing Factors of the Efficacy of Endoscopic Sinus Surgery in Patients with CRSwNP Complicated by Asthma

Abstract
Objective To explore the influencing factors of the efficacy of endoscopic sinus surgery in patients with chronic rhinosinusitis with nasal polyps(CRSwNP)complicated by asthma.Methods A total of 105 patients with CRSwNP complicated by asthma who underwent endoscopic sinus surgery in Yuncheng Central Hospital from June 2020 to October 2024 were prospectively enrolled.Clinical and surgical data of the patients were collected,and outcomes were evaluated 6 months after surgery.Based on the evaluation results,patients were divided into an effective group(89 cases)and an ineffective group(16 cases).Univariate and multivariate logistic regression analyses were used to analyze the influencing factors of surgical efficacy.Results The two groups showed no significant differences in gender,age,body mass index,side of lesion,American Society of Anesthesiologists(ASA)classification,history of alcohol consumption,history of smoking,course of CRSwNP,course of asthma,or history of previous sinus surgery(P>0.05).However,statistically significant differences were observed in Lund-Mackay score,Lund-Kennedy score,Visual Analogue Scale(VAS)score,proportion with concurrent allergic rhinitis,proportion of peripheral blood eosinophils(EOS),and tissue eosinophil(EOS)count(P<0.05).Multivariate logistic regression analysis showed that Lund-Mackay score,Lund-Kennedy score,VAS score,concurrent allergic rhinitis,proportion of peripheral blood eosinophils,and tissue eosinophil count were risk factors for surgical efficacy in patients with CRSwNP complicated by asthma(all P<0.05).Conclusion Implementing risk-stratified management based on these risk factors,combined with perioperative interventions to reduce risks and optimize individualized treatment plans,can effectively improve surgical safety and efficacy.关键词
