心肌酶谱联合PCT、CRP测定对肺炎支原体肺炎患儿病情程度的诊断价值分析
Diagnostic Value of Myocardial Enzyme Profile Combined with PCT and CRP Measurement for Disease Severity in Children with Mycoplasma Pneumoniae Pneumonia

Abstract
Objective To analyze the diagnostic value of myocardial enzymes combined with procalcitonin(PCT)and C-reactive protein(CRP)for the severity of Mycoplasma pneumoniae pneumonia(MPP)in children.Methods A total of 147 children with MPP admitted to the Department of General Pediatrics,Xuancheng People's Hospital from May 2023 to January 2024 were enrolled.PCT,CRP,and myocardial enzyme profiles were measured after admission.Based on clinical diagnosis,patients were divided into mild group(65 cases)and severe group(82 cases).General data,PCT,CRP,and myocardial enzyme levels[creatine kinase(CK),lactate dehydrogenase(LDH),aspartate aminotransferase(AST),and α-hydroxybutyrate dehydrogenase(α-HBDH)]were compared between groups.The diagnostic value of combining myocardial enzymes with PCT and CRP for MPP severity was analyzed.Results PCT and CRP levels were higher in the severe group than in the mild group(P<0.05).LDH and α-HBDH levels were higher in the severe group than in the mild group(P<0.05).Multivariate Logistic regression identified PCT(OR=11.140,95%CI:1.238-100.237),CRP(OR=1.097,95%CI:1.055-1.140),and LDH(OR=1.012,95%CI:1.000-1.024)as independent risk factors for severe MPP(P<0.05).ROC analysis showed AUC values of 0.658,0.740,0.654,and 0.639 for PCT,CRP,LDH,and α-HBDH alone,respectively.The combined detection AUC(0.831)was significantly higher than individual markers(Z=4.107,2.693,3.990,4.264,P<0.05).Conclusion Combined detection of PCT,CRP,LDH,and α-HBDH significantly improves diagnostic efficacy for MPP severity.PCT,CRP,and LDH are independent risk factors for severe MPP,providing reliable laboratory evidence for early identification of severity progression and clinical intervention guidance.关键词
