The Predictive Value of D-Dimer/Troponin Ⅰ Ratio for Major Adverse Cardiovascular Events in Overweight or Obese Emergency Community-Acquired Pneumonia Patients

Abstract
Objective To analyze the predictive value of D-dimer/troponin Ⅰ ratio(D-D/cTnI)for major adverse cardiovascular events in overweight or obese emergency community-acquired pneumonia(CAP)patients.Methods A retrospective analysis was conducted.Among the patients with CAP admitted to the First Hospital of Qinhuangdao from November 2022 to February 2025,46 patients who experienced major adverse cardiovascular events during hospitalization were included in the occurrence group,and 106 patients who did not experience major adverse cardiovascular events during hospitalization were included in the non-occurrence group.At admission,all patients had their D-dimer(D-D)and cardiac troponin I(cTnI)levels tested,and the D-D/cTnI ratio was calculated.General patient data and biochemical indicators were collected.The above indexes were compared between the groups.The predictive value of D-D/cTnI for major adverse cardiovascular events in emergency CAP patients was evaluated using the ROC curve and the AUC.Multivariate logistic regression analysis was used to analyze factors associated with major adverse cardiovascular events in emergency CAP patients.Results The occurrence group had a higher proportion of body mass index,coronary heart disease history,PSI score,peripheral blood D-D,cTnI,D-D/cTnI ratio,hs-CRP,and NT-proBNP levels,and lower oxygenation index(OI)and LVEF than the non-occurrence group(P<0.05).The results of the ROC curve showed that the AUC of D-D,cTnI and D-D/cTnI ratio in predicting major adverse cardiovascular events in emergency CAP patients were 0.844,0.850 and 0.910,respectively,and the AUC of D-D/cTnI ratio was significantly higher than those of D-D and cTnI(Z=6.266,8.469,P<0.001).Multivariate analysis showed that PSI score(OR=1.737),NT-proBNP(OR=1.560),and D-D/cTnI ratio(OR=3.766)were risk factors for major adverse cardiovascular events in emergency CAP patients,while OI(OR=0.426)and LVEF(OR=0.359)were protective factors(P<0.05).Conclusion An elevated D-D/cTnI ratio effectively predicts the occurrence of major adverse cardiovascular events in overweight or obese emergency CAP patients.关键词
