CT和MRI在髋关节直接前入路人工全髋关节置换手术中的应用价值
Application Value of CT and MRI in Artificial Total Hip Arthroplasty via Direct Anterior Approach

Abstract
Objective:To explore the application value of computed tomography(CT)and magnetic resonance imaging(MRI)in artificial total hip arthropasty(THA)through direct anterior approach(DAA).Methods:A total of 378 patients undergoing THA in Jiangxia District First People's Hospital,Wuhan,were enrolled between January 2019 and June 2023.According to different surgical approaches,they were divided into DAA group(n=192)and posterolateral approach(PLA)group(n=186).All patients underwent CT examination and MRI examination before surgery,at 1 month and 6 months after surgery.The perioperative data,CT and MRI parameters were compared between the two groups.The detection of complications and diagnostic efficiency of MRI were evaluated.Results:The operation time in DAA group was longer than that in PLA group,intraoperative blood loss was less than that in PLA group,and surgical incision was shorter than that in PLA group(P<0.05).The acetabular parameters by preoperative CT were consistent with those by intraoperative measurement(P>0.05),indicating that CT was conducive to the simulation of prosthesis implantation.MRI showed that width recovery of piriformis,musculi obturator externus and musculus obturator internus was better in DAA group than PLA group(P<0.05).There was no difference in the confirming of postoperative complications(aseptic loosening,osteolysis,periprosthesis infection)between MRI and surgical results(Kappa values=0.494,0.418,0.530,P>0.05).Conclusion:CT and MRI have important guidance value in perioperative period of THA through DAA.CT is conducive to the simulation of intraoperative prosthesis implantation,while MRI is beneficial to evaluate intraoperative muscle injury and diagnose postoperative complications.关键词
