摘要

Abstract

Objective:To explore the application effect of digital 3D reconstruction in accurate reconstruction of lower limb alignment in elderly patients undergoing hemiarthroplasty.Methods:A total of 108 elderly patients undergoing hemiarthroplasty were enrolled,and they were randomly divided into traditional group and 3D group,54 cases in each group.In traditional group,the film template was used for measurement,anteroposterior and lateral X-rays of pelvis were taken,and artificial planning was made according to morphology of hip joint,pubis and femur.In 3D group,digital 3D reconstruction technology was applied,and data were obtained by pelvic CT scan,then they were uploaded to 3D reconstruction planning system software to generate information,and fine tuning was performed by doctors accordingly to complete preoperative planning.The clinical indexes,lower limb alignment,hip function,imaging indexes,biochemical indexes and complications in the two groups were statistically analyzed and compared.Results:The operation time in 3D group was shorter than that in traditional group,and intraoperative blood loss was less than that in traditional group(P<0.05).At 3 months after surgery,lower limb alignment was decreased in both groups(P<0.05),Harris scores were increased(P<0.05),and improvement of the two indexes was more significant in 3D group(P<0.05).The deviations of abduction angle,anteversion angle,hip rotation center(horizontal)and hip rotation center(vertical)in 3D group were all lower than those in traditional group(P<0.05).At 3 days after surgery,levels of fibrinogen and D-dimer were increased in both groups(P<0.05),and the increase of the two indexes was lower in 3D group(P<0.05).There was no significant difference in the incidence of complications between the two groups(5.56%vs.11.11%,P>0.05).Conclusion:The application of digital 3D reconstruction is beneficial to accurately reconstructing lower limb alignment and accelerate recovery of elderly patients undergoing hemiarthroplasty.
