The Value of Amplitude Integrated Electroencephalogram Combined with Craniocerebral B-ultrasound and Craniocerebral MRI in Evaluating the Prognosis of Nervous System in Premature Infants during Neonatal Period

Abstract
Objective:The incidence of brain injury in premature infants is high,and the predictive value of traditional single imaging or electrophysiological examination for complex brain injury is limited.The purpose of this study was to explore the predictive value of combined application of amplitude integrated electroencephalogram(aEEG),craniocerebral B-ultrasound and MRI in the prognosis of nervous system in premature infants.Methods:A total of 180 preterm infants with gestational age≤36 weeks in the Sixth Affiliated Hospital,School of Medicine,South China University of Technology from November 2023 to May 2024 were enrolled.At 44 weeks of corrected gestational age,aEEG,craniocerebral B-ultrasound,and craniocerebral MRI were performed.According to the Gesell Development Scale(GDS)score at 6 months of corrected age,they were divided into poor neurological prognosis group(42 cases)and good prognosis group(138 cases).The abnormalities of aEEG,craniocerebral B-ultrasound,and MRI were compared between the two groups.Multivariate Logistic regression analysis was used to analyze the related factors affecting the development outcome of the nervous system in premature infants.The receiver operating characteristic(ROC)curve was drawn to analyze the value of aEEG,craniocerebral B-ultrasound and MRI in evaluating the development outcome of the nervous system.Results:The aEEG scores[(4.36±1.01)vs.(4.96±1.21),(5.64±1.28)vs.(7.06±1.34)]at 1 week after birth and 44 weeks of corrected gestational age in the poor prognosis group were lower than those in the good prognosis group,and the proportion of abnormal brain B-ultrasound and MRI was significantly higher than that in the good prognosis group(all P<0.05).Low aEEG score,high degree of brain B-ultrasound and MRI abnormalities were independent risk factors for poor prognosis(OR=3.725,4.125,3.695).The area under the curve(AUC)of the combined prediction of the prognosis of the nervous system in premature infants was 0.807,which was significantly higher than that of the single prediction(aEEG:0.771,craniocerebral B-ultrasound:0.673,MRI:0.666,P<0.05).Conclusion:The results of this study show that the combined application of aEEG,craniocerebral B-ultrasound and MRI can effectively predict the prognosis of nervous system in premature infants,which provides an important reference for early clinical intervention and is recommended for clinical promotion.Limitations of this study include the potential impact of phenobarbital sedatives used in MRI examinations on preterm infants,and the possible impact of limited sample size on the universality of the results.关键词
