Multimodal Magnetic Resonance Imaging in Assessing the Risk of Ischemic Stroke Caused by Non Stenotic Intracranial Atherosclerotic Plaque

Abstract
Objective:To explore the value of multimodal magnetic resonance imaging(MRI)in assessing the risk of ischemic stroke(IS)caused by non stenotic intracranial atherosclerotic plaque.Methods:The data of 178 patients with non-stenosis intracranial atherosclerotic plaques diagnosed from January 2021 to December 2023 were collected.Ingenia 3.0T CX MRI was used for multimodal imaging,including three-dimensional time-of-flight MRI,DWI,T2WI,T1WI scan,and then high-resolution MRI examination,T1WI scan,T2WI scan,and T1WI enhanced scan.According to whether IS occurred within 12 months of diagnosis,it was divided into the occurrence group(n=60)and the non-occurrence group(n=118).The basic data of patients and multimodal MRI parameters were statistically analyzed,and their evaluation value for IS risk was analyzed.Results:The plaque volume of the occurrence group was larger than that of the non-occurrence group,(50.03±10.72)mm3 and(36.89±11.05)mm3,respectively(P<0.001).The plaque enhancement rate of the occurrence group was larger than that of the non-occurrence group,(63.87%±6.55%)and(56.23%±5.13%),respectively(P<0.001).The area of the narrowest lumen in the occurrence group was smaller than that in the non-occurrence group,(3.58±0.58)mm2 and(4.73±0.63)mm2,respectively(P<0.001).The area under the receiver operating characteristic curve(AUC)of the prediction model based on plaque volume,plaque enhancement rate and the narrowest lumen area was 0.970,the sensitivity was 96.7%,and the specificity was 86.4%,indicating that the model had good predictive value.Conclusion:Multimodal MRI has certain evaluation value for the risk of IS caused by non-stenosis intracranial atherosclerotic plaques.Based on the risk prediction model constructed by the main parameters,the contribution of each factor to the risk of IS is intuitively expressed through the nomogram,which provides a quantitative evaluation tool for clinical practice.关键词
