Abstract

Objective:To study the effect of ultrasound Adler grading of blood flow in adnexal masses of ectopic pregnancy on the success rate of methotrexate treatment,providing reference for clinical treatment.Methods:A retrospective analysis of 357 ectopic pregnancy patients treated with methotrexate at Hefei Maternal and Child Health Hospital from January 1,2021 to December 31,2023 was conducted.Patients were divided into success and failure groups based on treatment outcome.General information,ultrasound indicators(including Adler grading),and serum HCG changes were compared.Binary logistic regression was used to assess the impact of blood flow grading on treatment success.Results:The success rate of methotrexate treatment was 72.27%.As the Adler grading of ultrasound blood flow grading increased,the success rate of treatment decreaseed significantly.Binary Logistic regression showed significant differences in treatment success from grades 0,1,2,to 3.The ORs were 16.546(95%CI:3.995~68.531),5.701(95%CI:1.452~22.395),4.382(95%CI:1.223~15.706),respectively(P<0.05).Conclusion:Ultrasound Adler grading of blood flow in ectopic pregnancy masses is a significant factor influencing the success rate of methotrexate treatment.Pretreatment blood flow grading,serum HCG levels and their changes are significant predictors of treatment success.
