改良隧道式腰大池引流对动脉瘤性蛛网膜下腔出血患者导管相关并发症的影响
Study on the complications of improved tunnel-type lumbar cistern drainage in the treatment of aneurysmal subarachnoid hemorrhage

Abstract
Objective:To evaluate the postoperative complications of modified tunnel lumbar cistern drainage for aneurysmal subarachnoid hemorrhage(aSAH).Methods:A retrospective analysis was conducted on the data of 86 aSAH patients admitted from October 2021 to December 2024.According to the cerebrospinal fluid drainage method,the patients were divided into traditional group and improved groups,with 43 cases in each group.In the traditional group,L3/4 or L4/5 lumbar puncture was performed according to the standard procedure,and the catheter was placed subdural about 10-15 cm,fixed with a dressing,and connected to a closed drainage system.The drainage height was maintained at 15-20 cm above the external auditory canal line.In the modified group,after successful catheterization as in the traditional group,a 0.5 cm incision was made at the puncture site,and a subcutaneous tunnel＞5 cm was established using a puncture needle.The catheter was placed in the subcutaneous tunnel and another incision was made to lead it out.The tunnel exit was fixed with a cross suture.The postoperative complications were observed in two groups of patients,including intracranial infection,catheter displacement,cerebrospinal fluid leakage,catheter blockage,and unplanned catheter removal.Results:The incidence of intracranial infection,catheter displacement,and unplanned catheter removal in the modified group was lower than those in the traditional group,with statistically significant difference(P＜0.05).There was no statistically significant difference in cerebrospinal fluid leakage and catheter blockage between the two groups(P＞0.05).Conclusion:The modified tunneled lumbar cistern drainage technique can effectively reduce the incidence of intracranial infection,catheter displacement,and unplanned catheter removal in aSAH patients.关键词
