Pathogenesis,early detection,diagnosis,and treatment of cisplatin-induced acute kidney injury

Abstract
Cisplatin-induced acute kidney injury(Cis-AKI)is a common complication during chemotherapy,characterized by a high incidence and complex mechanisms,which seriously affects patient treatment and prognosis.Its pathogenesis involves abnormal accumulation of cisplatin in renal tubular epithelial cells,metabolic toxicity caused by biotransformation,DNA damage,oxidative stress,inflammatory responses,and apoptosis.These pathological processes interact to ultimately impair renal function.Early detection and intervention are crucial for improving patient outcomes.Current methods for early detection of Cis-AKI include traditional indicators such as serum creatinine and blood urea;novel biomarkers such as kidney injury molecule-1,interleukin-18,and neutrophil gelatinase-associated lipocalin;as well as functional metabolomics and molecular imaging techniques,all of which have high sensitivity and specificity.In terms of treatment,strategies such as antioxidative stress,inhibition of cell death,alleviation of cell death,alleviation of inflammatory responses,and stem cell therapy have shown certain protective effects,but challenges remain in balancing efficacy with antitumor activity.Future research may focus on targeting key molecular pathways such as chemokine(C-X-C motif)ligand 16 and interleukin-22,using RNA interference and gene editing technologies to precisely regulate gene expression,integrating nanotechnology to achieve targeted drug delivery,and applying novel biomarkers and bioinformatics to enable precise diagnosis and individualized therapy.These efforts aim to optimize treatment strategies,improve clinical prognosis,and reduce the adverse effects of nephrotoxicity.关键词
