激光治疗颞下颌关节紊乱病的临床效果及机制初步研究
Preliminary study on the clinical effect and mechanism of laser treatment for temporomandibular joint disor-ders

Abstract
Objective:To investigate the effect of laser on temporomandibular joint disorders(TMD)by measuring the capsular width(CW)of the joint by high-frequency ultrasound,and the level of Asporin in the joint capsule.Methods:Thirty patients with temporomandibular joint disorders who were admitted to our department due to spontaneous pain and lim-ited mouth opening were randomly selected as the experimental group,and 30 healthy volunteers were selected as the control group.The experimental group was treated with semiconductor laser therapy(with a wavelength of 790～830 nm and a power of 250～300 mW),and the average width of bilateral joint capsule was measured by high-frequency ultrasound before treat-ment and 1 month after treatment.Compared with the control group,the synovial fluid of the two groups was collected at the same time.ELISA method was used to compare the level difference of synovial fluid Asporin between the two groups and the changes before and after treatment in the experimental group,and the changes of symptoms before and after treatment in the experimental group were recorded.Results:In the experimental group before laser therapy,the average width of the bilateral joint capsules under occlusal condition was 0.33 mm,and the average width at the maximum pain-free mouth opening was 0.43 mm.The difference was statistically significant compared to the control group(P<0.05).And the joint capsule width in the experimental group after 1 month of treatment was statistically significant compared with that before treatment(P<0.05).There were significant differences in Asporin levels between the experimental group and the control group and after laser treatment(P<0.05).The mouth opening and pain score of the experimental group were correlated with the joint capsule width.After laser treatment,the pain score was lower than before,and the mouth opening was significantly improved.Conclu-sion:Laser therapy demonstrates significant efficacy in treating temporomandibular joint pain,high-frequency ultrasound can be used as an effective tool to evaluae the treatment effect of laser.Asporin as a kind of inflammatory factors involved in the inflammatory response,for clinical diagnosis and explore the pathogenesis of temporomandibular joint disorders.关键词
