Abstract

Objective The aim of this study was to investigate the effects of different nutritional approaches on blood glucose,nutritional status,and immune function in patients with advanced esophageal cancer complicated with type 2 diabetes.Methods A prospective study was conducted on 126 patients with advanced esophageal cancer complicated with T2DM treated at the First Affiliated Hospital of Xinxiang Medical College from February 2022 to April 2024.The patients were divided into a control group(63 cases)and a study group(63 cases)according to the nutritional support pathway.Treatment for 7 days,blood glucose fluctuations,nutritional status,and immune function were compared between the two groups,inflammatory cytokine levels.Result After 1 day of treatment,the blood glucose fluctuation values increased in both groups(P<0.05),while the blood glucose fluctuation values decreased in the study group after 3 and 7 days of treatment(P<0.05),and were significantly lower than those in the control group(P<0.05).After 7 days of treatment,hemoglobin(Hb),albumin(ALb)in the study group decreased significantly(P<0.05),while Hb,transferrin(TRF),ALb,and prealbumin(PA)in the control group aslo decreased significantly(P<0.05).After 7 days of treatment,the differ-ences between the two groups in Hb,TRF,ALb and PA were statistically significant(P<0.05).Compared with before treatment,the levels of CD3+,CD4+and CD4+/CD8+in the study group significantly increased,while CD8+significantly decreased after 7 days tereat-ment(P<0.05).In the control group,CD8+decreased,while CD4+/CD8+increased significantly(P<0.05).The levels of CD3+,CD4+,CD4+/CD8+and CD8+in the study group were statistically significant differences(P<0.05).Compared with before treatment,the levels of interleukin-6(IL-6),IL-8,C-reactive protein(CRP),andtumor necrosis factor-α(TNF-α)decreased in both groups after 7 days of treatment(P<0.05).After 7 day of treatment,fthe differences between the two groups in IL-6,IL-8,CRP and TNF-αwere statistically significant(P<0.05).Conclusion Early use of enteral nutrition combined with intravenous nutrition for patients with advanced esophageal cancer and T2DM can reduce blood glucose fluctuations,improve patient nutritional status and immune func-tion,reduce levels of inflammatory cytokines.
