针刺联合重复经颅磁刺激治疗卒中后失眠的疗效观察
Observations on the efficacy of acupuncture plus repetitive transcranial magnetic stimulation for post-stroke insomnia

Abstract
Objective To observe the clinical efficacy of acupuncture plus repetitive transcranial magnetic stimulation(rTMS)for post-stroke insomnia(PSI).Method Sixty PSI patients were randomized to rTMS,acupuncture and combination groups,with 20 cases in each group.Each group was treated with basic treatment.The rTMS group received low frequency rTMS of right dorsolateral prefrontal lobe;the acupuncture group,acupuncture at Du meridian points;the combination group,acupuncture at Du meridian points plus low frequency rTMS of right dorsolateral prefrontal lobe.The Pittsburgh Sleep Quality Index(PSQI)score and sleep monitoring indicators(sleep efficiency,waking conversion frequency,sleep latency and the proportion of rapid eye movement(REM)sleep)were observed in the three groups before and after treatment.Result After treatment,the PSQI score decreased in the three groups compared with before(P＜0.05)and was lower in the combination group than in the acupuncture and rTMS groups(P＜0.05)and lower in the acupuncture group than in the rTMS group.After treatment,sleep efficiency,waking conversion frequency and sleep latency improved in the three groups(P＜0.05)and the proportion of REM sleep time increased in the combination and acupuncture groups(P＜0.05)compared with before;the proportion of REM sleep time had no statistically significant difference in the rTMS group compared with before(P＞0.05).After treatment,waking conversion frequency was lower in the combination group than in the acupuncture and rTMS groups(P＜0.05).After treatment,sleep efficiency,sleep latency and the proportion of REM sleep time were better in the combination group than in the rTMS group(P＜0.05)and sleep efficiency in the acupuncture group was better than in the rTMS group(P＜0.05).Conclusion On the basis of basic treatment,acupuncture plus rTMS can improve the quality and structure of sleep in PSI patients.关键词
