原发性高血压患者血清高迁移率族蛋白B1水平与早期肾功能损伤相关性研究
Correlation between serum high mobility group box 1 protein level and early renal injury in patients with essential hypertension

Abstract
Objective To analyze the correlation between serum high mobility group box 1 protein(HMGB1)level and early renal injury in patients with essential hypertension,and to explore its predictive value for early renal injury.Methods A total of 268 patients with essential hypertension admitted to Hengshui People's Hospital from June 2021 to September 2022 were enrolled.Based on urinary microalbumin-to-creatinine ratio(UACR)measurements,the patients were divided into an early renal injury group(UACR 30-300 mg/g)and a non-early renal injury group(UACR<30 mg/g).Differences in systolic blood pressure(SBP),diastolic blood pressure(DBP),HMGB1,systemic immune-inflammation index(SII),and renal function indexes were compared between the two groups.Correlation analysis were performed between HMGB1 and other parameters in the early renal injury group.Logistic regression was used to analyze the influencing factors for early renal injury in essential hypertension.Patients in the non-early renal injury group underwent 1.5-year follow-up and were divided into the new early renal injury subgroup and non-new early renal injury subgroup according to UACR.Baseline serum HMGB1 levels at admission were compared between the two groups.Receiver operating characteristic(ROC)curve was used to evaluate the predictive value of HMGB1 for new early renal injury.Results The early renal injury group demonstrated higher levels of SBP,HMGB1,SII,cystatin C(CysC),β2-microglobulin(β2-MG),α1-microglobulin(α1-MG),and UACR compared to the non-early renal injury group(t/Z=2.090,13.722,-5.536,10.688,3.659,-8.361,18.001;all P<0.05),along with lower estimated glomerular filtration rate(eGFR)(t=4.525,P<0.05).The duration of hypertension in the early renal injury group was longer than that in the non-early renal injury group(Z=-1.972,P<0.05).Serum HMGB1 in the early renal injury group was positively correlated with SBP,SII,CysC,β2-MG,α1-MG,and UACR(r=0.249,0.325,0.531,0.283,0.477,0.577;P<0.05),while negatively correlated with eGFR(r=-0.239,P<0.05).Logistic regression analysis showed that levels of SBP,α1-MG,CysC,and HMGB1 were risk factors for early renal injury(OR=1.026,1.188,16.528,1.643;P<0.05).Baseline serum HMGB1 and CysC levels in the new early renal injury subgroup were higher than those in the non-new early renal injury subgroup(t=10.060,3.029;P<0.05).ROC curve analysis revealed an AUC of 0.888 for HMGB1 in predicting new early renal injury.Conclusion Serum HMGB1 level is associated with early renal injury in essential hypertension and demonstrates predictive value for new-onset cases in non-early renal injury patients.Dynamic monitoring of HMGB1 shows clinical utility for identifying high-risk population.关键词
