Serum CA125,HE4 combined with BRCA gene status can be used to predict R0 resec-tion of interval debulking surgery in patients with advanced ovarian cancer

Abstract
Objective:To establish a model to predict R0 resection of advanced ovarian cancer with neoadjuvant chemotherapy combined with interval debulking surgery(NACT-IDS).Method:Clinical and pathological data were retrospectively collected from 327 patients who un-derwent neoadjuvant chemotherapy followed by interval debulking surgery at our institution be-tween 2010 and 2024.Univariate and multivariate analyses were performed to explore the rela-tionship between the clinical and pathological data of patients and the R0 resection rate of the IDS.A nomogram was constructed based on preoperative CA125,preoperative HE4,and BRCA gene status to predict whether R0 resection could be achieved.Result:The univariate analysis showed that the frequency of neoadjuvant chemotherapy increased,with KELIM≥1,preopera-tive CA125<137U/mL,preoperative HE4<116pmol/L,preoperative CA72-4<2.63U/mL,and BRCA mutations were identified as a favorable factor for achieving R0 resection.The multivari-ate analysis demonstrated that patients with preoperative CA125 was less than 137U/mL(OR=0.247,95%CI:0.083~0.738,P=0.012),preoperative HE4 was less than 116 pmol/L(OR=0.348,95%CI:0.135~0.905,P=0.031),and BRCA mutations(OR=4.411,95%CI:1.522~12.784,P=0.006)were more likely to achieve R0 resection.Combined with these three in-dicators,the AUC for R0 resection was 0.776(95%CI:0.692~0.860,P<0.001),with a sensitivity of 73.40%and specificity of 66.70%.Conclusion:Preoperative CA125,preopera-tive HE4 and BRCA status are valuable in predicting R0 resection following NACT-IDS.关键词
