Application Value of Transvaginal Ultrasound Combined with Transabdominal Ultrasound in the Early Diagnosis and Classification of Pregnancy at the Ce-sarean Incision Site

Abstract
Objective To investigate the application value of transvaginal ultrasound combined with transabdominal ultrasound in the early diagnosis and classification of pregnancy at the cesarean incision site.Methods A total of 64 pregnant women with suspected scar pregnancy after cesarean section admitted to Zhengzhou First People's Hospital from January 2023 to January 2025 were selected,and all of them received prenatal transvaginal ultrasound and transabdominal ultrasound examinations.Taking the pathological diagnosis results as the standard,the efficacy of the two ultrasound methods alone and in combination in the diagnosis of pregnancy at the cesarean incision site was compared,and their accuracy in differentiating the classification of pregnancy at the cesarean incision site was analyzed.Results The pathological diagnosis results showed that among 64 pregnant women,41 cases(64.06％)were diagnosed with pregnancy at the cesarean incision site,and 23 cases(35.94％)were normal pregnancy.Among the 41 women with pregnancy at the cesarean incision site,21 cases(51.22％)were of simple gestational sac type,14 cases(34.15％)were of mixed type,and 6 cases(14.63％)were of uterine cavity type.The accuracy of transvaginal ultrasound combined with transabdominal ultrasound in diagnosing pregnancy at the cesarean incision site was 98.44％,the sensitivity was 100.00％,the specificity was 95.65％,the positive predictive value was 97.62％,and the negative predictive value was 100.00％,all of which were higher than those of transabdominal ultrasound alone,while its accuracy was higher than that of transvaginal ultrasound diagnosis alone(P＜0.05).The Kappa value of the consistency between transvaginal ultrasound combined with transabdominal ultrasound result and pathological diagnosis result for pregnancy at the cesarean incision site was 0.856,higher than that of transvaginal ultrasound and transabdominal ultrasound alone(Kappa values were 0.719 and 0.637,respectively).For classification of mixed and uterine cavity types of pregnancy at the cesarean incision site,no statistically significant difference was found in the classification concordance rates between two ultrasound methods alone and in combination(P＞0.05);For simple gestational sac type classification,the classification concordance rate of two ultrasound methods in combination was significantly higher than that of two ultrasound methods alone(P＜0.05).Conclusions Transvaginal ultrasound combined with transabdominal ultrasound can improve the accuracy of early diagnosis of pregnancy at the cesarean incision site and the coincidence rate of simple gestational sac type,which is helpful to provide effective reference for the formulation of early treatment plan of pregnancy at the cesarean incision site.关键词
