经阴道二维超声联合经阴道三维超声对宫腔内占位性病变的诊断价值分析
Analysis on the Diagnostic Value of Transvaginal Two-Dimensional Ultrasound Combined with Transvaginal Three-Dimensional Ultrasound for Intrauterine Space-Occupying Lesions

Abstract
Objective To analyze the diagnostic value of transvaginal two-dimensional ultrasound(2D-TVS)combined with transvaginal three-dimensional ultrasound(3D-TVS)for intrauterine space-occupying lesions.Methods A total of 103 patients with suspected intrauterine space-occupying lesions attending Xinmi Hospital of Traditional Chinese Medicine from January 2022 to January 2025 were selected as the study objects,and all of them received 2D-TVS and 3D-TVS examinations.The detection results of intrauterine space-occupying lesions by 2D-TVS,and 2D-TVS combined with 3D-TVS were analyzed using hysteroscopic pathological biopsy results as the"gold standard";The imaging characteristics and blood flow parameters of patients with different benign intrauterine space-occupying lesions were compared.Results The results of hysteroscopic pathological biopsy showed that among 103 patients,68 cases were diagnosed with endometrial polyps(EP),25 cases were diagnosed with uterine fibroids,and 10 cases were diagnosed with endometrial cancer;The detection rate of EP by 2D-TVS combined with 3D-TVS was 61.17％,higher than 42.72％by 2D-TVS(P＜0.05),but no significant difference was found in the detection rate of uterine fibroids(21.36％vs.15.53％)and the detection rate of endometrial cancer(9.71％vs.7.77％)between 2D-TVS combined with 3D-TVS and 2D-TVS(P＞0.05).The endometrial thickness of the EP group was higher than that of the uterine fibroids group,and the lesion diameter was smaller than that of the uterine fibroids group(P＜0.05),but no significant difference was found in the characteristics of uneven echo(85.29％vs.88.00％),clear edge(91.18％vs.96.00％),regular shape(88.24％vs.92.00％)and rich blood flow(58.82％vs.40.00％)between the two groups(P＞0.05).The endometrial vascular blood flow index(VFI),vascular index(VI)and blood flow index(FI)in EP group were higher than those in uterine fibroids group(P＜0.05),but no significant difference was found in pulsatility index(PI)and resistance index(RI)of uterine artery and spiral artery between the two groups(P＞0.05).Conclusions 2D-TVS combined with 3D-TVS applied in the diagnosis of intrauterine space-occupying lesions,has high diagnostic efficacy for EP,and the combination of qualitative imaging characteristics and quantitative blood flow parameters is more helpful to identify EP.关键词
