Application of Revolution CT Whole-Brain Perfusion Combined with Head-Neck CTA in the Diagnosis,Treatment and Prognosis Evaluation of Patients with Acute Ischemic Stroke

Abstract
Objective To explore the application effect of Revolution CT whole-brain perfusion combined with head-neck CTA in the diagnosis,treatment and prognosis evaluation of patients with acute ischemic stroke(AIS).Methods 286 AIS patients admitted to our hospital from June 2021 to June 2023 were selected.All patients underwent combined examination of Revolution CT whole-brain perfusion and head-neck CTA.They were divided into good prognosis group(point 0～2,n=201)and poor prognosis group(point 3～5,n=85)according to the modified Rankin scale(mRS)score.The collateral circulation recovery status between the good prognosis group and poor prognosis group,the measurement results from different regions,and the stenosis status of culprit vessels in different CT whole-brain perfusion images were compared.Results The proportion of good collateral circulation recovery(ASITN/SIR score of 3～4)in the good prognosis group was 63.68％,significantly higher than 1.18％in the poor prognosis group(P＜0.05).The rCBF,rCBV,MTT and Tmax in the infarction core area were significantly different from those in the ischemic penumbra area and those in the mirror control area(P＜0.05);Significant differences were also found in the rCBF,MTT and Tmax between the ischemic penumbra area and the mirror control area(P＜0.05).The culprit vessel stenosis rate of patients with abnormal CT whole-brain perfusion images was significantly higher than that of patients with normal CT whole-brain perfusion images(P＜0.05).Conclusions Revolution CT whole-brain perfusion combined with head-neck CTA in the diagnosis,treatment and prognosis evaluation of AIS patients is helpful for formulating scientific treatment plans.关键词
