Detection of Serum 25-Hydroxyvitamin D Level in Children with Community-Acquired Pneumonia by Fully Automated Chemiluminescence Analyzer and Its Relationship with Immune and Inflammatory Markers

Abstract
Objective To investigate the serum 25-hydroxyvitamin D[25-(OH)D]level in children with community-acquired pneumonia(CAP)detected by fully automated chemiluminescence analyzer and its relationship with immune and inflammatory markers.Methods A total of 181 children with CAP admitted to our hospital from January 2024 to August 2024 were selected as case group,while 27 non-infectious disease children were selected as control group during the same period.Fasting venous blood(2 mL)was collected from all subjects,and serum 25-(OH)D concentration was measured using a fully automated chemiluminescence analyzer.The serum 25-(OH)D levels were compared between the two groups.181 CAP children were further divided into normal group,insufficient group,and deficient group based on their 25-(OH)D levels.The levels of inflammatory markers[C-reactive protein(CRP),procalcitonin(PCT),white blood cell count(WBC),erythrocyte sedimentation rate(ESR)]and immunoglobulins(IgA,IgM,IgG)were compared among the three groups.Independent sample t-test was used to compare intergroup differences,and Spearman rank correlation analysis was employed to explore the correlations between 25-(OH)D and immune/inflammatory markers.Results The 25-(OH)D level in the case group was(29.80±8.20)ng/mL,significantly lower than(36.82±9.21)ng/mL in the control group(P<0.05).Among the 181 CAP children,77 were in the normal group,70 in the insufficient group,and 34 in the deficient group.The levels of CRP,PCT,WBC,and ESR in the normal group were lower than those in the insufficient group and deficient group,and the insufficient group had lower levels than the deficient group(P<0.05).The levels of IgA,IgM,and IgG in the normal group were lower than those in the insufficient group and deficient group,and the insufficient group had lower levels than the deficient group(P<0.05).Spearman rank correlation analysis showed that 25-(OH)D level in CAP children was negatively correlated with CRP,PCT,WBC,ESR,IgA,IgM,and IgG(r<0,P<0.05).Conclusions The serum 25-(OH)D level decreases in children with CAP and is closely associated with immune dysfunction and hyperinflammatory responses.The fully automated chemiluminescence analyzer offers high efficiency and accuracy in detecting 25-(OH)D.Dynamic monitoring of this marker can assist in clinical assessment of disease progression and provide a basis for immunomodulatory and anti-inflammatory therapy.关键词
