伏立康唑相关低血糖的病例分析
Case report on voriconazole related hypoglycemia
摘要
Abstract
Objective To explore the clinical characteristics of voriconazolerelated hypoglycemia.Methods The diagnosis and treatment of two patients with voriconazole related hypoglycemia who were admitted to the Linyi people's hospital affiliated to shandong second medical university was reported,and the main clinical data(gender,age,disease prevalence,dosage of voriconazole,combined medication,incidence of hypoglycemia,clinical treatment and prognosis,etc.)of the two cases and similar cases collected by searching relevant databases(up to January 31,2025)were analyzed by descriptive statistic method.Results 9 patients(7 males and 2 females)were included in the analysis.The age ranged from 45 to 73 years,with an average age of 60±10 years and 4 patients ≥ 60 years.The indications for drug use were aspergillosis in 4 patients,fungal infection in 3 patients,fungal infection in 2 patients.There were 6 patients with diabetes mellitus,2 patients with liver cirrhosis,2 patients with hypoproteinemia and 2 patients with renal insufficiency.8 of 9 patients were co-administered with other drugs,and 1 patient did not report whether they were co-administered with drugs;8 patients had specific dosage of voriconazole,including 6 patients with 200 mg/time,1 time/12 h(double on the first day),1 patient with 180 mg/time,1 time/12 h(double on the first day),and 1 patient with 100 mg/d;8 of 9 patients had records of the time from voriconazole to hypoglycemia,ranging from 20 h to 30 d,with a median time of 2.5 d;8 patients had records of blood glucose level during hypoglycemia,ranging from 1.28 mmol·L-1 to 3.3 mmol·L-1,with an average of 2.11±0.59 mmol·L-1.9 patients had accompanying symptoms when hypoglycemia occurred,such as confusion,loss of consciousness,lethargy,coma,speech confusion,dizziness,sweating,etc.Glucose injection was infused immediately after hypoglycemia in all 9 patients,voriconazole was eventually discontinued in 7 patients,resumed 3 days after voriconazole discontinuation in 1 patient,and voriconazole was not discontinued in 1 patient.The blood glucose level of 9 patients recovered to normal after different treatments,6 patients had records of blood glucose level returning to normal,which was 18 h-8 d,with a median time of 1.69 d.Conclusion Voriconazole-related hypoglycemia mostly occurred within 5 days after medication,and it is mostly improved after discontinuation and glucose treatment.It is recommended that blood glucose levels be monitored during voriconazole administration.关键词
