Abstract

Background In<The interaction between muscle tone and other health-related factors in Children and Adoles-cents with Cerebral Palsy:A Systematic Review>(referred to as the<Muscle Tone Interaction Systematic Review>),the finding re-garding the interaction between muscle tone and gait was uncertain.This result did not entirely consistent with the judgments made by cerebral palsy experts based on clinical experience,necessitating further research to explore potential reasons.Objective To investigate the current state of research on the interaction between muscle tone and gait-related indicators in children and adoles-cents with cerebral palsy.Design Systematic review.Methods Data on muscle tone and gait dimensions were extracted from the<Muscle Tone Interaction Review>.Muscle tone indicators were categorized into three classifications based on the number of muscle groups involved(single muscle group,multiple muscle groups,and uncertain).Gait indicators were classified into three primary categories(spatiotemporal parameters,non-spatiotemporal parameters,and not applicable)and six detailed categories(temporal parameters,spatial parameters,kinematic parameters,kinetic parameters,electromyographic parameters,and not ap-plicable)according to their specific attribution to gait parameters.A constructed methodological framework was applied to deter-mine the interaction between muscle tone and gait indicator classifications.Main Outcome Measures The interaction between the classifications of muscle tone indicators and gait indicators.Results Data from 22 English-language studies involving 2,135 cases were included,with a mean sample size of 97(range:9-549).One study did not report the type of cerebral palsy,while the other 21 studies focused on pure spastic cerebral palsy.A total of ten types of muscle tone assessment indicators of the pyramidal system were cited,yielded 630 entries.Among them,16 studies involved single muscle group,including 20 single muscle groups with 559 entries;10 articles involved multiple muscle groups,including 9 sets of multiple muscle groups with 56 entries;and 2 ar-ticles involved uncertain muscle group,containing 15 entries.Regarding gait parameters,118 were mentioned,with a total of 630 entries.For the three-category classification:7 studies involved spatiotemporal parameters,17 parameters(14.4%),91 entries(14.4%);18 studies involved non-spatiotemporal parameters,100 parameters(84.7%),521 entries(82.7%),and 2 studies in-volved not applicable parameters,1 parameter,18 entries.For the six-category classification:7 studies involved temporal param-eters,11 parameters(9.3%),58 entries(9.2%);5 studies involved spatial parameters,6 parameters(5.1%),33 entries(5.2%);16 studies involved kinematic parameters,77 parameters(65.2%),344 entries(54.6%);1 study involved kinetic pa-rameters,13 parameters(11.0%),96 entries(15.2%);3 studies involved electromyographic parameters,9 parameters(7.6%),75 entries(11.9%);3 studies involved not applicable parameters,2 parameters(1.7%),24 entries(3.8%).The interaction be-tween muscle tone and gait indicator classifications was determined through the methodological approach.Conclusion In chil-dren and adolescents with cerebral palsy,the interaction between the three-category classification of muscle group-related muscle tone indicators and gait indicators was uncertain;the interactions between muscle tone indicators and the three-category as well as six-category classifications of gait indicators were uncertain,may not,or not.
