以视神经脊髓炎谱系疾病为首发症状的系统性红斑狼疮1例病例报告并文献复习
A case report and literature review of systemic lupus erythematosus presented as neuromy-elitis optica spectrum disorder

Abstract
We report a case of systemic lupus erythematosus(SLE)presenting as neuromyelitis optica spectrum disorder(NMOSD).A girl developed blurred vision at the age of 5 and was diagnosed with NMOSD based on positive AQP-4 antibody and abnormal head magnetic resonance imaging.Her condition improved after immunotherapy.At the age of 8,she developed thrombo-cytopenia.After treatment with methylprednisolone,cyclosporine,and plasma exchange,her platelet count returned to normal.At the age of 11,she developed fever and recurrent thrombocytopenia,accompanied by lymphopenia,anemia,hypocomplement-emia,positive antinuclear antibodies,positive anti-SSA,anti-Ro-52,and anti-SSB antibodies.She was diagnosed with SLE and treated with protein A immunoadsorption therapy,subcutaneous telitacicept injections,oral prednisone,and mycophenolate mofetil.After her platelets returned to normal,she was discharged and remained stable.We reviewed the literature and found that among SLE patients initially presenting with NMOSD,most have positive anti-DNA antibodies,and clinical manifestations mainly include arthralgia/arthritis,fever,abnormal blood counts,hypocomplementemia,and proteinuria.Some patients experience se-quelae such as visual impairment and limb paralysis.Clinicians should be aware of the possibility of NMOSD progressing to SLE.关键词
