Prognostication value of pallidum MRI combined with whole body movement quality assessment in neurodevelopment of neonates with severe hyperbilirubinemia

Abstract
Objective:To analyze the value of pallidum MRI combined with whole body movement quality assessment in predicting neurodevelopment of neonates with severe hyperbilirubinemia.Methods:A total of 86 neonates with severe hyperbilirubinemia who were treated in our hospital from August 2021 to August 2023 were collected as the observation objects.The newborns were followed up for one year.The development of the nervous system of the newborns was evaluated.According to the Bailey Scale of Infant and Toddler Development,the newborns were divided into the normal group(n=48)and the abnormal group(n=38).Compare the examination results of the two groups of newborns.Spearman correlation analysis was used to analyze the relationship between pallidum MRI and whole body movement quality assessment and neurodevelopment in neonates with severe hyperbilirubinemia.ROC curve analysis was used to evaluate the predictive value of pallidum MRI and whole body movement quality assessment for neurodevelopment of neonates with severe hyperbilirubinemia.Results:Neurodevelopmental results showed that 48 cases(55.81%)of the 86 neonates with severe hyperbilirubinemia had normal neurodevelopment,and 38 cases(44.19%)had developmental delay.The signal intensity of T1WI and T2WI in the pallidum region of the newborns in the abnormal group was significantly increased,and the difference was statistically significant(P<0.05).There were 46 normal neonates(53.49%)and 40 abnormal neonates(46.51%),among which 19(47.50%)neonates were diagnosed with mild brain injury,16(40.00%)with moderate brain injury and 5(12.50%)with severe brain injury.In the stage of restless movement,48 cases(55.81%)showed normal movement,31 cases(36.05%)showed lack of restless movement(F-),and 7 cases(8.14%)showed abnormal restless movement(AF).In the stage of twisting movement,46 cases(53.49%)showed normal movement,14 cases(16.28%)showed spasmodic synchrony general movement(CS),26 cases(30.23%)showed monotonic general movement(PR).Spearman correlation showed that pallidal MRI and whole body movement quality assessment were positively correlated with neurodevelopment.ROC curve analysis showed that the combination of pallidum MRI and whole body movement quality assessment had a high predictive value in detecting the neurodevelopment of neonates with severe hyperbilirubinemia.Conclusions:Both pallidum MRI and whole body movement quality assessment can be used to evaluate the neurodevelopment of neonates with severe hyperbilirubinemia,and their combined detection has higher predictive value.关键词
