4-Dimensional automated left atrial quantification technique for evaluating early changes after transcatheter aortic valve replacement

Abstract
Objective:To evaluate the early changes in left atrial volume and function after transcatheter aortic valve replacement(TAVR)using four-dimensional automated left atrial quantification(4D Auto LAQ)technology.Methods:This study included 43 patients who underwent TAVR at Sichuan Provincial People's Hospital between June 2020 and June 2022.All patients received echocardiography within 3 days pre-procedure and 3 months post-procedure.Standard 2D echocardiographic parameters were measured,and 4D Auto LAQ was used to assess left atrial volumes and strain.Parameters were compared before and after TAVR.Results:Three months after TAVR,peak aortic valve velocity(AV)in TAVR patients significantly decreased compared to preoperative values(P＜0.001),left atrial emptying volume index(LAEVI),left atrial passive emptying volume(LAVp),left atrial passive emptying volume index(LAVIp),left atrial ejection fraction(LAEF),and left atrial passive ejection fraction(LApEF)increased compared to preoperative values with statistical significance(P＜0.05),left atrial minimum volume(LAVmin),left atrial minimum volume index(LAVImin),left atrial maximum volume(LAVmax),left atrial maximum volume index(LAVImax),left atrial pre-atrial contraction volume(LAVpreA),left atrial pre-atrial contraction volume index(LAVIpreA),left atrial active emptying volume(LAVa),and left atrial active emptying volume index(LAVIa)decreased compared to preoperative values with statistical significance(P＜0.05),left atrial strain parameters all improved compared to preoperative values(P＜0.05),changes in early diastolic transmitral flow velocity(E),left atrial emptying volume(LAEV),and left atrial active ejection fractio(LAaEF)were not significant(P>0.05).Conclusion:4D Auto LAQ can effectively assess early postoperative left atrial volume and functional improvement in TAVR patients,holding promise as a reliable indicator for evaluating TAVR efficacy.关键词
