The study of evaluating myocardial work and the influencing factors in patients with complete left bundle branch block by two-dimensional speckle tracking echocardiography

Abstract
Objective:To evaluate the overall myocardial work in patients with complete left bundle branch block(CLBBB)by 2D speckle tracking echocardiography(2D-STE)and to explore the influencing factors.Methods:A retrospective analysis was conducted on 125 patients diagnosed with CLBBB who were admitted to our hospital from February 2020 to February 2023.One hundred and fifty healthy people matching were selected as control group(CON group).Linear regression analysis was performed on the parameters of left ventricular work.Results:Compared with CON group,CLBBB group had faster heart rate,higher blood pressure and longer QRS duration(P＜0.05).CLBBB group had left heart enlargement,left ventricular systolic function and diastolic function decreased,and systolic synchronization was worse(P＜0.05).LVGLS,GWE,GWI and GCW were decreased,Ts-SD was prolonged and GWW was increased in CLBBB group(P＜0.05).The work index of anterior septum,inferior septum and inferior wall myocardium in CLBBB group was significantly reduced.The myocardial work index of anterior wall and anterior lateral wall was significantly increased(P＜0.05).Ts-SD was independently associated with GWE,GWI,GCW and GWW in CLBBB group(all P＜0.05).Conclusion:In CLBBB patients,LVGLS,GWE,GWI,and GCW are decreased,Ts-SD is prolonged,and GWW is increased,regardless of whether LVEF is normal or not.Ts-SD is an independent influencing factor of decreased myocardial work.关键词
