CT-DSA融合成像在左心耳封堵术中的应用OA
Application of cardiac CT-DSA fusion imaging in left atrial appendage occlusion
目的:探讨CT-DSA融合成像在左心耳封堵术(LAAO)中应用的有效性和安全性.方法:收集110例行LAAO的患者,随机分为CT-DSA融合组(55例)和标准DSA组(55例).CT-DSA融合组在CT-DSA融合图像引导下行LAAO,标准DSA组在DSA引导下行LAAO.观察并对比2组手术成功率、对比剂用量、辐射剂量、手术时间、围手术期并发症及术后6个月的随访结局.结果:与标准DSA组相比,CT-DSA融合组的手术成功率更高(96.36%vs.83.64%,P=0.026),对比剂用量更少[(75.12±10.11)mL vs.(95.32±15.21)mL,P<0.001],辐射剂量更低[(2 515.25±124.33)mGy·cm² vs.(2 854.44±155.32)mGy·cm²,P<0.001],手术时间更短[(50.1±10.21)min vs.(60.35±12.17)min,P=0.02],6个月随访时卒中发生率更低(0 vs.5.45%).但2组围手术期并发症(心包积液、器械相关血栓和器械栓塞)发生率比较,差异均无统计学意义(均P>0.05).结论:与标准DSA成像相比,CT-DSA融合成像显著提高了LAAO的成功率和安全性.建议将CT-DSA融合成像整合到LAAO的标准实践中,以优化患者结局.
Objective:To investigate the efficacy and safety of CT-DSA fusion imaging in left atrial appendage occlusion(LAAO).Methods:A total of 110 patients undergoing LAAO were enrolled and randomly assigned to the CT-DSA fusion group(55 cases)and the standard DSA group(55 cases).CT-DSA fusion group underwent LAAO surgery guided by CT-DSA fusion images,while the standard DSA group underwent LAAO surgery guided by DSA.The procedural success rate,contrast agent usage,radiation dose,procedural duration,perioperative complications and follow-up outcomes at 6 months were compared between the two groups.Results:The CT-DSA fusion group showed a higher procedural success rate(96.36%vs.83.64%,P=0.026),less contrast media usage[(75.12±10.11)mL vs.(95.32±15.21)mL,P<0.001],lower radiation dose[(2 515.25±124.33)mGy·cm2 vs.(2 854.44±155.32)mGy·cm2,P<0.001],and shorter procedural duration[(50.1±10.21)min vs.(60.35±12.17)min,P=0.02],and lower incidence of stroke at 6 months(0 vs.5.45%)compared to DSA group.There were no significant differences in perioperative complications,including pericardial effusion,device-related thrombus,or device embolization(all P>0.05).Conclusions:CT-DSA fusion imaging significantly improves the success and safety of LAAO compared to standard DSA imaging.Integration of CT-DSA fusion imaging into standard practice for LAAO procedures is recommended to optimize patient outcomes.
