Abstract
Objective To systematically summarize the treatment and nursing care for the world's first patient with systemic lupus erythematosus(SLE)-associated refractory immune thrombocytopenia who under-went chimeric antigen receptor T-cell(CAR-T)therapy.Methods Based on routine care,the patient re-ceived specialized CAR-T treatment-related nursing interventions.During the collection phase,platelet counts were closely monitored,and three units of platelet support were administered to ensure safety.Key nursing measures during this stage included transfusion management and bleeding prevention strategies.Infection pre-vention and the management of adverse reactions to chemotherapeutic agents were prioritized during the eluvia-tion phase.During the backtransfusion phase,ensuring smooth infusion,monitoring vital signs,and observing urine output characteristics were critical.Post-CAR-T transfusion,the observation and management of adverse reactions constituted a key focus of nursing care.Following discharge,patients underwent one-year long-term fol-low-up,during which targeted health education and psychological support were continuously provided.Results Through standardized diagnosis,treatment,and meticulous nursing care,the patient's platelet count remained stable within the normal range after one year of follow-up post-CAR-T therapy.Conclusion CAR-T therapy for patients with SLE-associated refractory immune thrombocytopenia poses significant challenges and risks in terms of nursing care.However,close collaboration between medical and nursing teams can yield favorable outcomes,en-hancing both diagnostic and therapeutic efficacy as well as the patient's quality of life.关键词
