Abstract

Objective To analyze the correlation and diagnostic value of serum soluble triggering receptor expressed on myeloid cells-2(sTREM2)and insulin-like growth factor-1(IGF-1)in cognitive dysfunction among patients with type 2 diabetes mellitus(T2DM).Methods A total of 126 T2DM patients admitted to Wuhan Red Cross Hospital from January 2022 to October 2024 were included in the diabetes group,and 60 healthy examinees during the same period were included in the health group.General information and serum levels of sTREM2 and IGF-1 were compared between the two groups.According to the Montreal Cognitive Assessment Scale(MoCA)score,126 T2DM patients were divided into a normal group(65 cases)and an impaired group(61 cases).Logistic regression analysis was performed to identify influencing factors for cognitive dysfunction in T2DM patients.Spearman correlation analysis was used to assess the relationship between serum sTREM2,IGF-1 levels and MoCA scores.Receiver operating characteristic(ROC)curve analysis evaluated the diagnostic value of sTREM2 and IGF-1 for cognitive dysfunction in T2DM patients.Results Serum sTREM2 levels were higher in the diabetes group than in the healthy group,while IGF-1 levels were lower(both P<0.05).The impaired group had longer disease duration(P<0.05),higher fasting blood glucose(FPG),heart rate,glycated hemoglobin(HbA1c),low-density lipoprotein cholesterol(LDL-C),fasting insulin(FINS),C-reactive protein(CRP),and sTREM2 levels(P<0.05),but lower high-density lipoprotein cholesterol(HDL-C),IGF-1,and MoCA scores(P<0.05)compared to the normal group.Longer disease duration,elevated HbA1c,FINS,CRP,and sTREM2 were risk factors for cognitive dysfunction in T2DM patients,while elevated IGF-1 was a protective factor(all P<0.05).Serum sTREM2 was negatively correlated with MoCA scores(r=-0.447,P<0.001),whereas serum IGF-1 was positively correlated(r=0.413,P<0.001).The area under the curve(AUC)for combined detection of serum sTREM2 and IGF-1 in diagnosing cognitive dysfunction(0.938)was higher than that of individual detection(0.808 and 0.794,respectively;P<0.05).Conclusions T2DM patients exhibit elevated serum sTREM2 levels and reduced IGF-1 levels.Compared to those without cognitive dysfunction,patients with cognitive dysfunction show higher sTREM2 and lower IGF-1 levels.Combined detection of serum sTREM2 and IGF-1 demonstrates higher diagnostic value for T2DM-associated cognitive dysfunction.
