The Relationship between Myoglobin and Septic Shock in Patients with Sepsis and Its Prognostic Value

Abstract
Objective To explore the relationship between myoglobin(MYO)and septic shock in patients with sepsis and its prognostic value.Methods A retrospective study was conducted on 348 sepsis patients admitted to the Affiliated Hospital of Sichuan Nursing Vocational College from September 2021 to January 2025.They were divided into a non-shock group(253 cases)and a shock group(95 cases)based on the occurrence of septic shock.According to the 28-day prognosis,they were further classified into a 28-day survival group(247 cases)and a 28-day death group(101 cases).General clinical data were collected,and serum MYO levels were measured.Pearson correlation analysis was used to examine the relationship between serum MYO levels and lactate,Acute Physiology and Chronic Health EvaluationⅡ(APACHEⅡ)score,and Sequential Organ Failure Assessment(SOFA)score.Multivariate Logistic stepwise regression analysis was applied to identify influencing factors for prognosis in sepsis patients.The evaluation value of influencing factors on the prognosis of patients with sepsis was analyzed using the ROC curve.Kaplan-Meier survival curves were used to compare the 28-day survival rates between patients with high and low serum MYO levels.Results The shock group had higher levels of lactate,APACHEⅡscore,SOFA score,and MYO compared to the non-shock group(P<0.01).Serum MYO was positively correlated with lactate,APACHEⅡscore,,and SOFA scores in sepsis patients(r=0.219,0.252,0.296,respectively;P<0.01).The 28-day death group showed higher age,CRP,Scr,lactate,APACHEⅡscore,SOFA score,and MYO levels than the 28-day survival group(P<0.05).Multivariate Logistic stepwise regression analysis indicated that lactate,APACHEⅡscore,SOFA score,and MYO were independent prognostic factors in sepsis patients(OR=10.290,95%CI:3.948-26.820,P<0.01;OR=1.419,95%CI:1.256-1.602,P<0.01;OR=1.334,95%CI:1.060-1.678,P=0.014;OR=1.009,95%CI:1.006-1.012,P<0.01).ROC curve analysis revealed that the combination of lactate,APACHEⅡscore,SOFA score,and MYO for predicting prognosis in sepsis patients had an AUC of 0.936,which was significantly better than that of lactate,APACHEⅡscore,SOFA score,or MYO alone(AUC=0.715,0.758,0.685,0.837,respectively;P<0.01).Kaplan-Meier survival curve analysis showed that the high MYO patients had a lower survival rate than the low MYO patients(P<0.05).Conclusion Elevated MYO is closely associated with the development of septic shock and poor prognosis in sepsis patients.The combination of serum MYO,APACHEⅡscore,and SOFA score has high predictive value for the prognosis of sepsis patients.关键词
