彩色多普勒超声联合血清ADAM12、NR3C2检测对乳腺癌早期诊断的价值分析
The Value of Color Doppler Ultrasound Combined with Serum ADAM12 and NR3C2 in the Early Diagnosis of Breast Cancer

Abstract
Objective To explore the value analysis of color Doppler ultrasound combined with serum a disintegrin and metalloproteinase 12(ADAM12)and nuclear receptor subfamily 3 group C member 2(NR3C2)in the early diagnosis of breast cancer.Methods Patients with early-stage breast cancer admitted to the Second Affiliated Hospital of Xinjiang Medical University from September 2022 to December 2023 were selected as the observation group(96 cases),and patients with benign breast lesions during the same period were selected as the control group(78 cases).The levels of serum ADAM12 and NR3C2 were compared between the two groups.The ROC curve was used to analyze the diagnostic value of serum ADAM12 and NR3C2 levels for early breast cancer.The fourfold table method was used to analyze the diagnostic value of color Doppler ultrasound combined with serum ADAM12 and NR3C2 levels for early breast cancer.Results The proportions of irregular shape,spiculation,blurred margins,posterior echo attenuation,microcalcifications,and grade Ⅱ-Ⅲ blood flow in the observation group were higher than those in the control group(P<0.05).The serum ADAM12 level in the observation group was higher than that in the control group(P<0.05),while the NR3C2 level was lower(P<0.05).Color Doppler ultrasound combined with serum ADAM12 and NR3C2 levels showed a malignancy predictive value of 82.30%and a benign predictive value of 95.08%,consistent with the gold standard(Kappa value=0.727,P<0.05).The diagnostic results of color Doppler ultrasound,ADAM12,and NR3C2 were consistent with the gold standard(Kappa values=0.579,0.580,0.552,P<0.05).The sensitivity,specificity,and accuracy of the combined method were 96.88%,74.36%,and 86.78%,respectively.The sensitivity of the combined diagnosis was higher than that of color Doppler ultrasound,ADAM12,or NR3C2 alone(P<0.05);its specificity was lower than that of color Doppler ultrasound alone(P<0.05);and its accuracy was higher than that of color Doppler ultrasound or NR3C2 alone(P<0.05).Conclusion Serum ADAM12 is up-regulated and NR3C2 is down-regulated in early breast cancer patients.The accuracy of color Doppler ultrasound combined with serum ADAM12 and NR3C2 levels is higher than that of color Doppler ultrasound or NR3C2 alone,which can effectively improve early diagnostic accuracy and provide certain reference for clinical screening,diagnosis,and treatment.关键词
