涤痰汤化裁方治疗慢性阻塞性肺疾病合并肺部感染疗效及对血清IL-6、IFN-γ及TNF-α水平的影响
Efficacy of Modified Ditan Decoction(涤痰汤化裁方)in Treatment of Chronic Obstructive Pulmonary Disease Complicated with Pulmonary Infection and Its Effect on Serum IL-6,IFN-γ and TNF-α

Abstract
Objective To explore the clinical efficacy of Modified Ditan Decoction(涤痰汤化裁方)in the treatment of chronic obstructive pulmonary disease(COPD)complicated with pulmonary infection and its effect on pro-inflammatory cytokines inter-leukin-6(IL-6),interferon-γ(IFN-γ)and tumor necrosis factor-α(TNF-α)in serum.Methods One hundred and twenty-four patients with chronic obstructive pulmonary disease and lung infection in the hospital from January 2022 to June 2023 were randomly divided into study group(62 cases)and control group(62 cases).The control group was treated with con-ventional Western medicine.Based on this,the study group received Modified Ditan Decoction.Then the clinical efficacy,TCM syndrome scores,blood oxygen partial pressure(PaO2),blood oxygen saturation(SaO2),arterial blood carbon dioxide partial pres-sure(PaCO2),forced expiratory volume in the first second(FEV1),maximal voluntary ventilation(MVV),forced vital capacity(FVC),the ratio of forced expiratory volume to forced vital capacity in the first second(FEV1/FEV2),interleukin-6(IL-6),interferon-γ(IFN-γ)and tumor necrosis factor-α(TNF-α)were compared between two groups.Results No statistical difference was reported between two groups in terms of baseline pulmonary function parameters,blood gas index,TCM syndrome scores,or the levels of serum IL-6,IFN-γ and TNF-α(all P＞0.05).The total efficacy rate was 96.77％in study group,which was higher than 87.10％in control group(P＜0.05).The study group had lower TCM syndrome scores than the control group after treatment(P＜0.05).After treatment,the blood gas indexes of the two groups,such as PaO2 and SaO2 increased and PaCO2 decreased,and the improvement in the study group was more obvious(P＜0.05).After treatment,lung function indexes such as FEV1,MVV,FVC and FEV1/FVC in both groups increased,and the improvement in the study group was more obvious(P＜0.05).After treatment,the serum levels of IL-6,IFN-γ and TNF-α in the two groups decreased,and the improvement in the study group was more obvious,with statistical significance(P＜0.05).Conclusion Application of Modified Ditan Decoction in the treatment of chronic obstructive pulmonary disease complicated with pulmonary infection can effectively ameliorate the pul-monary function,alleviate the clinical symptoms,and reduce the levels of IL-6,IFN-γ and TNF-α in serum.关键词
