489例肝细胞癌患者生存特征及影响因素分析:一项回顾性队列研究
Survival characteristics and influencing factors of 489 patients with hepatocellular carcinoma:a retrospective cohort study

Abstract
Objective:To investigate the survival characteristics and influencing factors of patients with hepatocellular carcinoma(HCC).Methods:The medical records of all patients with liver cancer who were hospitalized in Hubei Hospital of Traditional Chinese Medicine from December 2014 to May 2022 were retrospectively analyzed.The study compared the survival characteristics of patients based on gender,age,comorbidities,tumor staging,and different treatment modalities,and analyzed the risk factors affecting survival.Results:A total of 850 HCC patients were admitted to the hospital during the study period,and the complete survival of 489 HCC patients was followed up,with the longest survival period of 229.15 months and the shortest survival period of nine days.The median survival time of the 489 patients was 19.99 months.The overall cumulative survival rates at 1,3,5,and 10 years were 60.7%、39.9%、29.4%,and 22.7%,respectively.The median survival times for patients at Barcelona Clinic Liver Cancer(BCLC)stages A(0),B,C,and D were 133.25 months,41.95 months,7.79 months,and 2.79 months,respectively.Univariate Cox regression analysis showed that there were differences in the survival time of HCC patients of different age groups,with a history of viral hepatitis,type 2 diabetes,different BCLC stages,and different treatment modalities(P<0.05).Multivariate Cox regression analysis showed that being aged 50-59 years compared to40-49 years(HR=1.504,95%CI 1.045-2.164,P=0.028),having cirrhosis at the initial diagnosis(HR=1.509,95%CI 1.168-1.949,P=0.002)had statistically significant impacts on survival time.In comparison to stage B,stage A(0)(HR=0.293,95%CI 0.196-0.439,P<0.001),stage C(HR=4.333,95%CI 3.184-5.898,P<0.001),and stage D(HR=9.636,95%CI 6.603-14.062,P<0.001)had statistically significant impacts on survival time.Compared to conventional Western medicine treatment,combined traditional Chinese and Western medicine treatment(HR=0.557,95%CI 0.424-0.731,P<0.001)and the general treatment group(HR=2.173,95%CI 1.527-3.094,P<0.001)had statistically significant impacts on survival time.Conclusion:The overall survival time of HCC patients is relatively short.Being aged 50-59 years,having cirrhosis at the initial diagnosis,being at BCLC stages C/D,and receiving only symptomatic supportive treatment increase the risk of survival for HCC patients.However,being at BCLC stage A(0)and receiving combined traditional Chinese and Western medicine treatment can prolong the survival of patients.关键词
